- Observation (CORRECTED DURING INSPECTION): [REPEAT VIOLATION] Frozen individually packaged raw red meat observed stored in the chest and display coolers. Some packages are observed stored in black garbage bags. No label available for meats. Operator is not able to provide invoice for meats to confirm if the meat is from an approved source during time of inspection. Risk of selling meat from unapproved source was discussed with operator. 32 packs of frozen red meat were voluntarily discarded by the operator during time of inspection.
- Corrective Action(s): DO NOT OBTAIN meat from unapproved sources. All foods must be obtained from an approved supplier to prevent foodbone illness to customers. Invoices must be received upon receiving all food and kept on site for verification by the health inspector. Failure to comply in future may lead to further progressive enforcement (Corrective/Closure Order and/or ticketing).

- 1. Some boxes of foods and bags of dry foods observed stored directly on the floor during inspection.
- 2. Operator has installed bulk food containers on a shelving unit at the front of the store without submission of updated plans to and review/approval from the district health inspector. Operator placed a sign on the bulk containers reading "not for sale" during time of inspection.
- Corrective Action(s):
- 1. All foods must be stored at least 6 inches off the floor and covered at all times to prevent contamination.
- 2. Cease sales of all bulk food items until you have submitited plans to the health inspector for review AND the plans have been reviewed and approved by the health inspector. Failure to comply may lead to progressive enforcement.

- Observation: 79 cartons of free range eggs (one carton has 30 eggs) were stored at room temperature on the shelves. Internal temperature of egg was measured at 17.5C. Cold potentially hazardous food such as eggs must be stored at a temperature of 4C or less.
- Corrective Action(s): Eggs are HELD in storage room for 1 day. A follow up inspection to be conducted on April 21, 2022. See violation 208 for details.

- Observation: 1. Frozen individually packaged raw meat - beef, fish & chicken was found in the freezers. No labels were found on the packages. Operator stated that the meat was bought from a nearby farm. No receipts/invoices provided to confirm if the supplier is an approved source. The risk of selling food from unapproved source was discussed and the operator VOLUNTARILY discarded 26 packages of frozen raw meat during the inspection.
- 2. 79 cartons of free range eggs (1 carton has 30 eggs) were observed at the room temperature. Operator stated that the eggs were bought from nearby free range farm. No grading of eggs was evident. No invoices/receipts available to verify the supplier. The box of eggs states to store eggs at 18C. Operator was told to discard the eggs. However, operator would like to return it to the supplier. Invoice/receipt would be required to confirm the return of eggs.
- Corrective Action(s): DO NOT SELL the eggs to the customers. Operator removed the eggs from the shelves and moved them to another room during inspection. Eggs are HELD for 1 day. A follow-up inspection will be conducted.

- 1. Bottom shelves used for storing takeout container boxes
- 2. Along the wall near cash counter
- 3. Behind chest freezer
- Corrective Action(s): Remove mouse droppings and disinfect the area with 1 part bleach to 9 parts water solution. Use gloves and proper PPE during cleaning. Regularly monitor for any mouse activity in the store. Discuss the issue with the contracted Pest Control Company to implement pest control measures.
