- Client contact information was collected and retained as required.Sterility assurance documentation was not collected.Corrective Action:- Going forward, ensure sterility assurance documentation for all critical equipment is collected and retained with client records.
- 26. Are cosmetic products appropriate for their intended use and being handled properly?
- Neuromodulators (Dysport) were reconstituted with bacteriostatic saline and used for a maximum of 2 days.Previously used, single use vials of bacteriostatic saline were stored unlabeled in the refrigerator. Corrective action:- Ensure manufacturer's instructions for use (MIFU) is followed. Reconstitute with preservative free saline and store between 2-8C for up to 24 hours. Unused portion after that time must be discarded.- Obtain a thermometer so the temperature in the refrigerator can be monitored.- Ensure all single use vials are discarded after one use.- For multi-dose vials, labeled the date of opening, and discarded according to manufacturer's instructions, or 28 days after opening (if no manufacturer beyond use date is specified), whichever is shortest.
