- Operator said that skin was cleansed but that antiseptic was not always used before an invasive service.-Operator must use a skin antiseptic prior to a service that punctures skin-Written procedures now reflect the fact that operator will use an alcohol wipe on the skin after cleansing
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Operator was not keeping sterility information in client records.-Sterility assurance information must be maintained with client forms for all critical equipment used during a service-Operator will begin to maintain information by attaching sterile packages to client consent forms
