- 1-PreEmpt CS20- high level disinfectant passed it expiry date of Feb 2024. Discard the expired disinfectant and obtain a disinfectant with valid expiry date to disinfect the semi-critical tools. Owner was advised to send picture of the bottle with expiry date to PHI via text message ASAP. number provided.
- 20. Is reusable equipment disinfected properly?
- 1-High level disinfectant CS20 test strips were expired, passed their expiry date of Feb 2024. The test strips were discarded onsite. Obtain test strips with valid expiry date or discard the high-level disinfectant at the end of the day.Owner advised PHI that CS20 has been changed daily and facility do not reuse the high-level disinfectant next day.2-No high-level disinfectant testing record was available in the facility for review. Operator reported no reuse of high-level disinfectant. High level disinfectant is changed daily. No need to do the testing and keeping test record.3-Bubbles were observed in the CS20 dish. The solution was discarded and the dish was washed and left on counter to dry.4-Operator reported contact time for CS20 for 15-20min. The label of the CS20 container was reviewed during onsite and contact time of disinfectant was explained to the operator. Ensure all disinfectants are used in accordance to the manufacturer instructions for use.
