      • There were no written procedures for skin cleansing or accidental skin cut/puncture.     • 09. Is a point of service risk assessment being performed of the condition of the client and of the equipment to be used on the client prior to performing a personal service?
      • Nail clippers were rusty and can no longer be appropriately cleaned and disinfected. Please ensure rust is removed or nail clippers are replaced.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Client's skin was not being cleansed before waxing services are provided. Please cleanse the client's skin before providing waxing services with soap and water or baby wipes.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Porous uncleanable equipment was being re-used between clients. Equipment such as nail files and buffer blocks are porous and cannot be properly disinfected. Please dispose of uncleanable equipment between each client.
    • 15. Is reusable equipment handled and stored properly (Critical)?
      • Contaminated nail clippers and used dremel covers were left at the workstation after use. Please ensure contaminated equipment is moved to the reprocessing area in a timely manner after use to ensure it is easily identifiable from clean equipment and not re-used between clients.
    • 20. Is reusable equipment disinfected properly?
      • 1. Semi-critical equipment was not being disinfected properly because the high-level disinfectant (Pre-Empt CS20) expired Nov 8, 2021. Please purchase new high-level disinfectant for use TODAY as this poses a serious risk to public health. 2. Proper records were not being maintained of high-level disinfectant testing. The last test was done July 22, 2025. Please ensure the high-level disinfectant is tested daily and the results recorded.3. Intermediate-level disinfectant (PreEmpt RTU) expired in March 2025. Please get new, non-expired disinfectant.

      • Some products (like Lotus) did not have appropriate labels including directions for use. Please obtain full labels for these products including directions for use.

    • 20. Is reusable equipment disinfected properly?
      • 1- Inadequate and inconsistent records High-level disinfectant (HLD) was noted.2- HLD was not being tested every day before use.3- HLD was being used in accordance to manufacturer instruction for use. HLD poured in the container failed the test during inspection. The HLD-CS20 was discarded during inspection.Education provided to all operators. Ensure the HLD is used according to manufacturer instruction for use and daily tested before use and record is maintained. In the event operator is unable to test the HLD every day, the solution must be discarded at the end of the day and new pour is required every day.
