Client records must include:- Full client name- Contact information of client- Date(s) of personal service- Description of personal service- Name of personal services worker who provided the personal serviceCorrective Action:Please provide copy of client record form for personal services that use critical equipment (such as microneedling)

- A mobile sink is used for a purpose other than for handwashing or reprocessing. - A mobile sink must not be used in a personal service facility except for hand washing and for the reprocessing of reusable equipment used in the provision of personal services other than reusable equipment described in section 6(1).This means a mobile sink (such as the one you have set-up):1. Would only be permitted for handwashing purposes OR reprocessing of reusable equipment that is non-critical (touches only hair or intact skin).2. Would not be permitted for reprocessing of reusable equipment that is semi-critical (comes into contact with mucous membrane or non-intact skin) or critical equipment (punctures skin or enters sterile tissues of the body).Corrective Action Options (non-exhaustive list):- Install a plumbed in sink.- Use an existing plumbed in sink somewhere else in the home that would be dedicated to the personal service business. It cannot be located in an area with incompatible activities such as sleeping, dining, food preparation, a room with a toilet, etc. - Not provide services that require reprocessing of semi-critical or critical equipment. - Use only single use/disposable semi critical or critical equipment, eliminating the requirement to reprocess equipment.- Finding a commercial space to operate out of that meets the sink requirements.
