    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Upon discussion with a technician, it was observed that sterility documentation for the micro needling packages was not retained on client records.Sterility documentation located on the back of the micro needle packages must be included in all client records when micro needling is performed.
    • 20. Is reusable equipment disinfected properly?
      • The facility uses HLD5 for disinfection of semi critical equipment such as nail trimmers. HLD5 is re-used for up to 14 days as per the manufacturer's instructions. Germinex RTU (intermediate level disinfectant) is used to disinfect the pedicure tubs. Low-level disinfectant (Lysol wipes) available for disinfecting surfaces. Three violations observed related to disinfection practices:1. The jar used to submerge larger items vertically is labeled with HLD5 disinfection procedures. However, the instructions state to soak the tools for three minutes. Five minute contact time is required as per the manufacturer's instructions. Please change the instruction label and ensure all relevant equipment is fully submerged in the HLD5 for at least five minutes before rinsing and air drying. 2. Test strips for the HLD5 were expired as of September 2025. Please purchase new test strips for the HLD5. 3. High-level disinfection test logs were not available at the time of the inspection. A template was provided at the front desk at the end of the inspection. A copy of this template will also be emailed. The HLD5 must be tested daily and the test results must be recorded on a log.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • Multiple cosmetic bottles and jars were observed to be unlabeled, primarily at the manicure and pedicure stations. All cosmetic products must be labeled.
