- Observation: Permit was not posted in an area visible to the public. Public Health Significance: Permit not being posted in a conspicuous location denies the public from making informed decisions about the facility and its ability to maintain required standards of sanitation and food safety.
- Corrective Action: Operator to ensure permit is posted in a conspicuous location.
- 209 - Food not protected from contamination [s. 12(a)]
- Observation: (CORRECTED DURING INSPECTION) Food items (meat and chicken wings) were stored on the floor of the walk-in freezer. Public Health significance: Storing food on the floor exposes food to contamination from physical, biological and chemical hazards which can cause foodborne illness if such food is consumed.
- Corrective Action: Ensure all food items are stored 15cm off the floor always.

- Observation: No written food safety plan on-site. Public Health Significance: The absence of such document on-site creates a gap in knowledge when employees need to make reference to procedural steps on how to mitigate food safety hazards in the food premises.
- Corrective Action: The operator is required to educate employees on proper food safety practices. The operator must also ensure that a written food safety plan and sanitation plan are readily available on site and accessible to employees.

- Observation: Food debris and frozen blood from meat were observed on the floor of the walk-in freezer. Public Health significance: Accumulated food debris serves as a breeding ground for pathogens which can cross-contaminate/contaminate food items, which could lead to foodborne illness if consumed.
- Corrective Action: Operator to ensure the walk-in freezer is cleaned and maintained in sanitary condition.

- Observation: (CORRECTED DURING INSPECTION) Sanitization step was not observed while using a 3 compartment sink for dishwashing. Public Health significance: Washing and rinsing plates without sanitizing them encourages pathogens to survive, multiply and spread, which could lead to foodborne illness if meal is served and consumed on such plates.
- Corrective Action: Educated employees on dish washing steps for a 3 compartment sink and referred employees to the manufacturer's rinse instructions of the Chlorinated disinfectant solution on site.
