- In the walk-in cooler, the fan cover had a build-up of dust and debris. -Clean the fan cover to prevent any further possible cross-contamination.

- In the back of house, by the shipping and receiving door, the floor panels was lifted and exposing the raw wooden floor underneath. -Fix the floor to create a proper smooth, non-absorbent, and easily cleanable surface. Wood is not a proper surface.

- In the walk-in cooler, duct tape was on the handle had a build-up of debris.-Ensure all surfaces are smooth, non-absorbent, and easily cleanable. Duct tape is not an acceptable surface.

- In the back door was duct taped around the entire frame and had a build-up of debris. Possibly the purpose is for insulation. -Ensure all surfaces are smooth, non-absorbent, and easily cleanable. Duct tape is not an acceptable surface.
