      • Sections of flooring are cracking and/or lifting.May 5, 2026 update:There are small divots in several areas of flooring in the main play area.There is a larger hole in one corner of the play space.All flooring must be smooth, impervious to moisture, and easy to clean.Repair/seal the floor so it is impervious to moisture, or replace flooring.Provide EPH with a plan by June 22, 2026 on how the floor will be brought into compliance before the beginning of the 2026/2027 school year.

    • 06. Are proper protocols being followed in regards to client care?
      • Saber hydrogen peroxide based disinfectant is in use.Test strips are not available.Ensure test strips appropriate to the type of chemical in use are present. Test strips must be used to verify disinfectant concentration when solutions are prepared, and at least daily if solutions are used over multiple days. It is best practice to record the disinfectant concentration.

      • Bingo daubers specifying "keep out of reach of children" were present.Concerns were discussed with operator and daubers were set aside to be removed.Ensure bingo daubers and other craft supplies are non toxic and appropriate for use by children.

      • 1. An unpleasant urine-like odor was detected in the boys' washroom.Investigate the source of the odor and mitigate as necessary.Note: there is a radiator present in the washroom, which may have been urinated on/in.2. An odd clanging sound was detected coming from the radiator in the boys' washroom. Investigate the source of the sound and ensure the radiator is functioning properly.3. There is a hole beside the toilet paper dispenser in the boys' washroom.Ensure the hole is repaired and that the surfaces are smooth, impervious to moisture, and easy to clean.

      • There was no thermometer present in the fridge to monitor temperature.Temperature of the fridge measured 2.4C with the inspector's thermometer.Ensure the fridge has an internal thermometer present to monitor temperature, which must be maintained at 4C or less.
