- 03. Are clients at this facility being protected from hazards (Critical)?
- Following concerns were noted:1) Some arts and craft materials like glue and bubble soap were observed in the toddler room accessible to children.2) Small toy blocks (possibly choking hazard) were observed in the infant room.3) Gardening tools with pointed edges were observed in the pre-school room.4) Push pins were observed on the notice board in pre-school room.Concerns were addressed during the inspection.**Ensure to store arts/ craft materials and gardening tools inaccessible to children and to be used only under adult supervision**
- 04. Are clients at this facility being protected from hazards (Non-Critical)?
- 1) The staff bathroom had two stalls (one of them was also used as storage are) the main door to the bathroom stalls could not be locked properly.Install an appropriate locking mechanism or child gate to ensure the bathroom is inaccessible to children.2) Staples were observed on notice boards in the childcare rooms. Cover all exposed staples with plastic tape to eliminate the risk of injury.3) An electrical outlet in the pre-school area (near the window) was found without a protective cover. Install a childproof cover on the identified outlet to prevent electrical hazards.4) The back board of a shelving unit facing towards the infant area had nails and the back board was not fully covering the back of shelving unit cleaving a gap that could be potential entrapment hazard.Ensure to repair or replace the indicated shelving unit.

- Following points were discussed regarding the appropriate diapering procedures as per AHS guidelines:1) Use of disposable gloves and hand hygiene before redressing the child.2) Use of paper liner for diapering surface Operator confirmed that diapering procedures will be reviewed with staff.
- 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
- 1) The side panel of the shelving unit (between the infant and toddler room) was unfinished at the top.Ensure to repair the shelving unit so that its in smooth and easily cleanable condition.2) Highchairs in infant room and kitchen play set in the toddler room were observed in unclean condition, with food debris and accumulated grime.Ensure to clean and sanitize the indicated toys and equipment.3) Paint was peeling off from ceiling in corner of infant room. (Scrape off the flaking paint to make the area smooth and to prevent further damage. Water staining was also observed around the indicated wall. it is highly recommended to investigate any underlying water leak or infiltration).4) Loose threads were coming out from the area rug in toddler room.Repair or replace the indicated area rug.5) Drilled holes were observed in the walls in pre-school room (near the exterior window).Ensure that all the walls are maintained in suitable manner.** Discussed clean in place method for big toys, furniture and play equipment using two-step process:1) Cleaning with soap and water.2) Sanitizing with spray bottle (100-200ppm bleach solution).

- 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
- Sleeping cots were stored at a diagonal angle against a bookshelf, with the bedding slightly touching the floor and the cot beside. Sleeping cots and blankets must be stored in a manner that prevents contamination and allows for separation between children. This was corrected during the inspection.
- 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this child care facility (Non-Critical)?
- The roll of paper towel used for drying hands in the children's washroom was stored too close to the handwashing sink and placed directly on the countertop. This setup presents a risk of splash contamination during use.Please install paper towel dispensers in all washrooms. Dispensers should fully enclose the paper towels to maintain hygiene and prevent contamination.

- Some stuffed animals were dirty. - Laundry soft items weekly or more often if necessary.
