- Observation: 2 dead cockroaches noted in a glue trap placed under the cash register beside the computer.
- Corrective Action(s): There has been an on-going cockroach problem within the premises. Operator has an on-going contract with a pest control company organized through mall management.
- Coordinated insecticide treatments have been done between mall management (for common areas), the restaurant owner, and other food vendors within the food court area to rectify the cockroach problem. On-going regular insecticide treatment will be needed to reduce and rectify the pest problem.

- Observation (CORRECTED DURING INSPECTION): Old encrusted lettuce was noted on the slicer. Tomato slicer was not properly washed and sanitized as the was remains of food debris.
- Corrective Action(s): Ensure slicers are properly washed and sanitized after use. A fine scrubbed brush may need to be use to clean and sanitized the space between the blades.Slicers were re-washed and re-sanitized at the time of inspection.
