- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- Written procedures were not available during the inspection. - Ensure all required written procedures are readily available onsite to personal service workers including the cleansing of skin and mucous membranes, application of antiseptic products, post-service care for personal services that involve puncturing the skin or mucous membrane, client care in the event of an accidental skin cut or puncture, and decontamination of any sink that will be used for both handwashing and equipment reprocessing.
- 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
- The 70% isopropyl alcohol prep pads used as an antiseptic prior to piercing were not labelled with a DIN or NPN. - Ensure the antiseptic used by the facility has a DIN or NPN. The operator obtained a 70% isopropyl alcohol prep pads with an NPN during the inspection.
- 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
- Sterility documentation and corresponding client records for previous piercing services were not available at the time of inspection.- Ensure sterility assurance documentation, including the piercing cartridge lot number, is kept with the required client records for the client that the equipment was used for. The operator demonstrated appropriate sterility documentation and client record retention process during the inspection. The operator created a reference template for sterility documentation and client records including client full name, contact information, date of service, description of service, and full name of the service provider with critical equipment lot number for staff reference.
- 15. Is reusable equipment handled and stored properly (Critical)?
- The Inverness piercing system was stored in a cloth bag with visible debris. - Store reusable equipment in a manner that protects from contamination. The operator removed the inverness piercing system to be stored in a container/bag that can be easily cleaned and disinfected.
- 20. Is reusable equipment disinfected properly?
- Debris build up was noted at the bottom of a Barbicide solution container. - Ensure to follow manufacturer's instructions for use of Barbicide disinfectant, which requires preparing a fresh solution daily or more often if the solution becomes diluted or soiled. Fresh Barbicide solution was prepared by the operator.
- 20. Is reusable equipment disinfected properly?
- An appropriate disinfectant was not available for disinfecting the Inverness piercing system as the equipment is to be "wiped" during cleaning and disinfection as per the manufacturer's instructions for use. - Obtain a low-level disinfectant wipe, at minimum, with a short contact time, such as 4 minutes or less, that is indicated for use on this type of equipment. An intermediate level disinfectant wipe with a shorter contact time is recommended.

- An appropriate disinfectant for the clippers was not available onsite. - Obtain a low-level disinfectant, at minimum, that is intended for use on clippers. The operator obtained Clippercide during the inspection.
- 26. Are cosmetic products appropriate for their intended use and being handled properly?
- Berrywell and Refractocil hair dyes reported to previously be used for eyebrow tinting both indicated the dyes are not to be used on the eyebrows or eyelashes. - Ensure to follow the manufacturer's instructions for use of all cosmetic products. The operator discarded the dyes.
