      • In the 12 months to 3 years room, the diapering change pad is not equipped with a safety strap.Please add a safety strap that is easy to clean and in good repair.In compliance:Diapering supplies is labeled for the client to which is belongs, and is protected from contamination.Diapering stations are co-located with dedicated handwashing sinks.Diapering surfaces are well maintained and easy to clean.Diapering areas are distinctly separate from food areas.1000ppm chlorine solution used for sanitizing surfaces at diapering stations.
    • 24. Are sleeping supplies and areas sufficient to ensure client safety and comfort (Non-Critical)?
      • Staff are seeking alternative options for linen-cot storage, claiming that adding and removing the linens each day is time consuming. Currently, linens are stored in labeled reusable shopping-style bags, which are hung on labeled hooks. The hooks are located directly above flush toilets and is co-located with diapering. These hooks are located on the opposite side of the room from the cot storage.Flushing toilets aerosolizes the contents of the toilet up to 6 feet in all directions. The health concern is that the reusable bags are contaminated with bacteria from the flushing toilets, and that these bags are contacted prior to and following handling of linens and cots.Please relocate the linen storage so that it is not co-located with toilets and diapering.Discussed with operator cubbies and pantry-style cupboards with doors that can be locked. PHI in conversations with operator regarding suitable alternative storage methods for linens.Note: storing cots pre-made with linens is not acceptable for preventing contamination of client personal effects.

      • The facility provides a toothbrush for each client. These items are stored out of client's reach. Each toothbrush is equipped with a cover for the bristles and is stored vertically in a labeled compartment with the client's name and picture. This is satisfactory.One toothpaste tube is used to deposit toothpaste on each toothbrush. The health concern is that the nozzle of the toothpaste tube serves as a contamination point from one toothbrush to another, effectively contaminating all toothbrushes with each other and perpetuating the spread of disease.Discussed several options with operator including the following:-Eliminate the use of toothpaste and emphasize the physical removal of debris via manual brushing without chemical assistance.-Assign one (1) labeled tube of child-grade toothpaste per child.-Acquire a pump-action style dispenser for the toothpaste (similar to a pump-action hand soap container) where the risk of brush-to-dispenser contact is a minimal risk.-Acquire child-grade tablet toothpaste (one tablet per client).-Utilize a toothpick or cotton swab to move toothpaste from the squeeze tube nozzle to the toothbrush, discarding the toothpick/cotton swab after each toothbrush.Please let me know how this process will be adjusted going forward.

      • Thumbtacks are being used to hold arts and crafts in the childcare rooms, several of the thumbtacks were observed to be loose.**Thumbtacks are a potential source of injury to clients, please have the thumbtacks removed.

      • A staff member was observed to be using gloves improperly. Staff member did not wash their hands or use an ABHR after removing the gloves used for attending to a client with a fecal accident.

      • A chair and table with their legs in disrepair was observed in the Toddler room during the inspection.**Ensure all furniture is in good repair to prevent possible injury.-The operator removed the pieces of furniture from the childcare area during the inspection.

      • A loose cable from a lamp not in use was accessible to the clients in the Toddler room. **Ensure loose cables and cords are stored in a manner that is inaccessible to the clients.

      • Client personal items such as jackets were observed to be in contact with each other.**Ensure personal items are stored in a manner that allows for distinct separation of each client's items.
