    • 03. Are clients at this facility being protected from hazards (Critical)?
      • 1. There were tall shelving units that were not bolted to the wall in the classrooms. - Bolt all tall shelving to the wall to prevent them from falling.2. Push pins were used on the classroom's notification boards.- Please secure notices using tape, as push pins may pop out and cause a risk of injury.
    • 08. Is this facility properly constructed (Non-Critical)?
      • A double sliding door in the hallway and another in the Excellers washroom had thin or inadequate sealant, making them prone to water absorption.Apply another coat of sealant or paint the doors. All surfaces must be cleanable and non-absorbent
    • 17. Is the facility and equipment being maintained in a suitable manner (Critical)?
      • Diapering mats in all rooms were observed to be torn in multiple areas and repaired with tape. In some classrooms, two mats were stacked, material under the top mats were of absorbent.- Remove all damaged. Replace with single piece, non absorbent, and easily cleanable diapering mats that are maintained in good condition.

      • 1. There were water damaged ceilings tiles in the Expander and Explorer rooms.- Replace the water damaged ceiling tiles.2. A play couch in the Expenders room was observed to be torn in multiple areas, rendering the surface not cleanable.Repair the torn couch, ensure the surface is smooth, non-absorbent and easy to clean.

      • A pacifier was observed stored on a shelving unit. No designated container was available for storing mouthed toys.Ensure pacifiers are individually labeled and stored in a clean, sanitary manner. Provide a clearly labeled, washable container designated for mouthed toys.
