- Spatulas used for handling of cooked rice was placed directly above the lid.Operator was directed to remove it from the lid and store it in the appropriate way (handle in upright position/ not in direct contact with food) to avoid any cross contamination.

- The roof was noted to be leaking at multiple locations (on top of the grill, on top of the dishwashing sinks, on top of standup cooler. Please repair the leaking ceiling. The operator indicated that they would require a little timeframe for this repair. A tarpaulin was noted to be covering the roof. According to the video, the roof was repaired. This would need to be confirmed during an onsite inspection.

- 1. All food storage bulk containers were noted to be in insanitary state. Please clean and sanitize all bulk food containers. 2. Cardboard and nonfood related containers were being used to store food items. Please acquire food grade containers to store food items. 3. Nonfood related items were stored with food related items. Please designate an area to store nonfood related items. Please do not store nonfood related items with food items.

- It was noted that the facility was not following proper manual dishwashing procedures.Proper manual dishwashing procedures were discussed with the operator during the inspection. Please follow following procedures:- Wash with soap and water and rinse with clean water in the first sink.- Prepare bleach solution at 100 ppm in the second sink and leave clean dishes fully submerged in the second sink for at least two minutes. Air dry all dishes.

Please clean and sanitize the facility thoroughly and ensure it is maintained in a clean and sanitary manner at all times. Please create a written cleaning schedule.
