- There was no designated hand sink nor hand washing soap in the kitchen. Soap from the front service area sink was provided. Ensure there is a designated hand sink, and all hand sinks are fully equipped with running hot and cold water, soap and paper towel at all times.

- Used cleaning cloths were not stored properly when not in use. They were observed sitting out on counter tops in the kitchen.All used cleaning cloths were immediately stored in a freshly made 200ppm Quat solution by manager upon request by PHI trainee.A quats solution of 200ppm and all cleaning cloths were stored in the sanitizer.

- The kitchen chef was observed preparing and handling food in the kitchen without a hair restraint.The chef immediately wore a hair net upon request by PHI trainee.

- There is inadequate separation of food items from non-food items (chemicals and staff personal belongings) in the storage area beside the dishwasher.-Reorganize the storage area beside the dishwasher and have all food items well separated from non-food items to prevent potential food contamination.

- Raw chicken in a container was stored above and beside cook food items and vegetables in the walk-in cooler.The operator rearranged the walk-in cooler upon request by PHI trainee and the raw chicken was stored below ready-to-eat foods, fruits and vegetables in the walk-in cooler.

- A Ziploc bag with raw chicken was observed in a sink at room temperature. The kitchen chef indicated that it was kept overnight at room temperature for thawing.- The chicken was immediately discarded by the operator upon request by PHI trainee. Proper methods to thaw frozen foods were discussed with staff.
