      • Observation:The toilet seat in the male washroom was observed to be loose and not securely fastened.Corrective Action Required:Securely fasten the toilet seat to ensure it is stable and properly affixed. If the seat cannot be adequately secured, replace the toilet seat to ensure it is maintained in good repair.

      • Observation:Slime (biofilm) buildup was observed on the internal surfaces of the ice machine.Corrective Action Required:Thoroughly clean and sanitize the ice machine in accordance with the manufacturer’s instructions to remove all biofilm buildup. Maintain the ice machine in a clean and sanitary condition at all times.

      • Observations:Reoccurring Violation (September 19, 2025): A buildup of dust was again observed on the ceiling tiles and vents in the main food preparation area, indicating failure to maintain the area in a sanitary condition.Accumulation of food debris was observed in crevices and hard-to-reach areas throughout the kitchen.Corrective Actions Required:Conduct a thorough deep cleaning of all kitchen areas, including ceiling tiles, vents, crevices, and hard-to-reach surfaces, to remove dust and food debris. Develop, implement, and maintain an effective cleaning and monitoring schedule to prevent recurrence and ensure ongoing compliance with sanitary maintenance requirements.
