- 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
- Facility-specific written procedures were not available for services that puncture the skin and for sink decontamination where a sink may be used for both handwashing and equipment reprocessing. Please develop and maintain facility-specific written procedures for cleansing skin and applying antiseptic products before applicable services, post-service care following services that puncture the skin or mucous membrane, and response to accidental skin cuts or punctures. Please also develop written procedures describing sink decontamination steps where a dual-purpose sink is used.
- 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
- The sink used for handwashing in the tattoo service room was not set up with handwashing supplies in a practical manner to support routine hand hygiene, as liquid soap and paper towels were not mounted/provided at the sink in an appropriate manner. Please ensure the handwashing sink is properly supplied and set up to support routine hand hygiene during tattooing and other personal service activities.
- 20. Is reusable equipment disinfected properly?
- Reusable semi-critical instruments were not being disinfected properly because the disinfectant manufacturer's instructions were not being followed. The operator advised that semi-critical instruments are immersed in a ready-to-use disinfectant solution and that the same solution is reused for up to seven days, despite manufacturer instructions indicating the solution must be discarded after each use. Semi-critical equipment must be thoroughly cleaned, rinsed, and dried prior to disinfection, then disinfected using at least a high-level disinfectant, with the disinfectant used exactly in accordance with manufacturer instructions, including contact time, reuse period, storage, and rinsing. Please ensure all reusable semi-critical instruments are properly cleaned prior to disinfection and that the disinfectant is used exactly as directed by the manufacturer. Please keep the manufacturer's instructions available on site for review.
- 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
- 1. The manufacturer's instructions for the steam sterilizer were not readily accessible at the time of inspection. Sterilizers must be operated in accordance with manufacturer's instructions, and those instructions must be readily accessible to personal services workers. Please ensure the manufacturer's instructions for the sterilizer are available on site for reference by personal services workers at all times.2. Sterilization monitoring could not be adequately verified. The operator advised that sterilization parameters are handwritten manually; however, no reliable method was demonstrated to corroborate that required physical parameters are being achieved for each load, and required biological monitoring was not demonstrated. Before the steam sterilizer is put into use for body piercing jewelry or other critical equipment, please ensure the sterilizer is operated in accordance with manufacturer's instructions and that required physical, chemical, and biological monitoring is completed, verified, and documented for each applicable load.
- 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
- The Alma Soprano XL class 4 diode laser could not be shown to be operated in accordance with applicable requirements. Manufacturer's instructions were not readily accessible at the time of inspection, and the operator could not demonstrate compliance with manufacturer instructions for use, operator training requirements, room-entry warning signage requirements, and applicable device documentation, including registration information for the class 4 laser. Please ensure the device is not used unless manufacturer's instructions are available on site, staff are trained in its safe operation, required warning signage is in place, and applicable registration/device documentation can be provided for review.
- 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
- 1. The personal service facility was not maintained in good repair. A damaged/broken baseboard was observed in the tattoo service room. Please repair the damaged baseboard and maintain the facility in good repair.2. The facility did not have adequate storage space for equipment and supplies used in invasive personal services. Needles, cartridges, dermal rollers, lancets, tubes, and other related supplies were observed stored in a crowded and disorganized manner, with items placed on top of one another in a way that may compromise package integrity and increase the risk of contamination. Please provide sufficient storage and organize all invasive service supplies so that sterile and single-use items are protected from damage and contamination, maintain package integrity until point of use, and are clearly separated from other supplies.
- 30. Is the reprocessing area of a personal service facility that uses reusable equipment properly designed, managed and maintained to ensure the safety of clients?
- Body piercing must not commence until an acceptable system is established for sterilization of non-sterile body jewelry. The operator advised that, at this time, only commercially purchased jewelry received non-sterile would be sterilized, and no dirty instruments are currently being reprocessed for body piercing. If sterilization is to occur at another personal service facility, that facility must have a compliant reprocessing area and the written procedure must address transport, handling, sterilization, sterility assurance, and storage. If any additional reusable equipment or instruments are to be reprocessed in the future, please contact AHS before those activities begin.
