- 1. Foods are at risk of contamination from the handwashing sink located at the main line due to its close proximity to ready-to-eat food. Install a splash guard on the handwashing sink to prevent contamination during handwashing.2. Dust and food build up was observed on bulk food storage containers. Conduct cleaning to all dirty food storage containers.

- Bulk food scoops were observed stored on top of the food containers. All food utensils must be stored in a sanitary manner, away from dirt and potential pests, to prevent contamination of both the utensils and the food.

- A positive sanitizer test was not achieved from the dishwasher. Conduct servicing to the dishwasher to ensure it is operating effectively for proper sanitization.

- The insides of the walk-in cooler have heavy buildup of dust on the ceiling and fan.

- The primary hand sink beside the prep cooler requires a partition. Ensure the partition material is smooth, impervious to moisture, easy to clean and in good repair.

- Mouse droppings were observed in the liquor storage room still outstanding. The area adjacent to the liquor room had minor droppings.

- Please continue to inspect all exterior doors and close off any gaps with proper materials. At the time, there were gaps in between exterior double doors.

- Both hand sinks at the bar were missing paper towels. Operator was looking for some during the inspection.

- Mouse droppings were observed in the liquor storage room, the area adjacent to the liquor room. The ceiling in the liquor storage room is open and should have ceiling tiles installed to prevent entry into the facility. Remove the droppings and repair any holes, cracks, gaps. Also please keep the area clean. Pest control reports showed findings of activity (flies and mice) as well. Some flies noted in the liquor room. Please investigate to determine where the issue is such as spills, drains, leaks, etc. Fly strips were placed near the back prep area and the in front of the liquor room. Please relocate the fly strips to an area away from food or food contact surfaces.

- Weatherstripping in certain areas were in poor repair (ie. front entrance). Please inspect all exterior doors and close off any gaps with proper materials.

- The insides of the prep coolers and some of the handles had buildup of food and grease.The deep fryer and the surrounding area had buildup of food and grease.The ceilings and the condenser fans of the walk-in units had some buildup of dust. Operator indicated that some renovations were due in the near future and would address those areas.The dry ingredients bins (w/ blue lids) had containers stored inside. Please keep them stored in a manner that prevents cross contamination.The white mobile bins (holding dry ingredients) had buildup of debris, please clean

- Signs of multiple flying insects noted around each bar glasswasher. Ensure the glasswashers are properly cleaned and drained. Recommend discussing with dishwasher technician about what parts can be disassembled at the end of the night and/ or cleaned differently.

- There are gaps in the weatherstripping at the back door which could allow for easy access to pests.Ensure weatherstripping is in good repair and covers gaps underneath the door.

- - Exhaust canopy noted to have grease buildup on some filters.- White cutting board on hot side prep cooler was discolored, grooved, and did not appear easily cleanable. - Dishracks appeared dirtyClean the above areas regularly and ensure regular cleaning of these areas are on the cleaning schedule. Ensure the white cutting board is smooth and easily cleanable, including removal of dirt/ stains that might accumulate.
