    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • AHS written procedure templates were left onsite with the operator. The operator was asked to review and complete the following applicable written procedures:• Cleansing of skin and mucous membrane• Application of antiseptic products• Post-service care for personal services that involve puncturing the skin or mucous membrane• Client care in the event of an accidental skin cut or puncture• Decontamination of any sink that will be used for both handwashing and equipment reprocessingThis violation will be corrected upon the receipt of completed written procedures.
    • 12. Are personal services workers at the facility maintaining client records and where applicable, sterility documentation corresponding to that client?
      • Sterility assurance documentation from critical equipment is not being kept with client records.1. Permanent makeup tattooing needle cartridges 2. Microneedles used for microneedling Please ensure to attach sterility assurance documentation (lot number & manufacturer contact) with client records in the case that issues arise after a personal service is performed and to know which associated instruments/items to recall.
    • 28. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Critical)?
      • Cistern is used for potable water for home/business and is located in the front of the property. A recent water sample had not been submitted for this facility. Please note that according to the new Public Health Guidelines for Non-Municipal Drinking Water, quarterly water samples should be submitted for all personal services facilities where the water source is a cistern. The operator had been unaware of the sampling requirements and has been asked to submit a water sample. This violation will be corrected upon the receipt of a satisfactory bacteriological water sample.
