- At time of inspection:1. The front entrance door was not properly sealed at the bottom to help ensure pests were not easily capable of entering this facility.2. There did not appear to be any pest control records for this facility.The owner / operator of this facility is requested to conduct the following, namely:1. Re-seal the bottom of the front entrance door.2. Ensure the AHS pest control checklist is filled out once per month and kept in a binder so that they can be reviewed during future inspections.
