- Observation (CORRECTED DURING INSPECTION): Less than 200 ppm QUATS sanitizer was measured in the sanitizer spray bottle in the front service area. Spray nozzle of sanitizer spray bottle was not in good working order.
- Corrective Action(s): Staff member re-stocked the sanitizer spray bottle with 200 ppm QUATS sanitizer and replaced the sanitizer spray nozzle during the inspection.

- Observation (CORRECTED DURING INSPECTION): 1. Paper towels were not available at the handsink near the prep.coolers and cook line.
- 2. Handsink was obstructed with preparation utensils near the prep. sinks.
- Corrective Action(s): Ensure handsinks are accessible, not blocked, and kept supplied with liquid soap, hot and cold running water, and single-use paper towels to enable proper handwashing practices. Person in charge re-stocked paper towels at the handsink near the cook line, and removed the preparation utensils from the second handsink and placed them into the ware-washing sink.

- Observation (CORRECTED DURING INSPECTION): Floor mat was observed stored inside one of the two compartment sink units.
- Corrective Action(s): Manager removed the floor mat, and washed, rinsed, and sanitized the sink unit. Discontinue practice of storing floor mats in the ware-washing or prep sinks to protect food from any potential contamination.

- Observation (CORRECTED DURING INSPECTION): Paper towels were not accessible from the wall-mounted paper towel dispenser at the handsink in the back area. Dispenser was stocked with paper towels.
- Corrective Action(s): Manager fixed the dispenser to enable access to the single-use paper towels. Ensure handsinks are supplied with accessible liquid soap, hot and cold running water, and single-use paper towels for handwashing.

- Observation (CORRECTED DURING INSPECTION): 1. Glassware and cutlery were observed stored on a cabinet in a customer accessible area without being covered or without an appropriate sneeze-guard barrier in place.
- 2. Tomato sauce was observed stored in an open metal can in the walk-in-cooler.
- Corrective Action(s): 1. Ensure service utensils are stored in a manner to protect them from potential contamination. Staff inverted the glassware on the storage cabinet surface, and saran wrapped the cutlery container to protect the service utensils from any potential contamination (e.g. potential coughing or sneezing by customers).
- 2. Ensure food contents of open metal cans are stored in food containers to protect food from any potential contamination. Manager transferred the tomato sauce into a stainless steel food container during the inspection.

- Observation (CORRECTED DURING INSPECTION): Dishwasher final rinse chlorine sanitizer residual was at 0 ppm at the plate level. Chlorine sanitizer level appeared low in the connected sanitizer pail.
- Corrective Action(s): Dishwasher final rinse chlorine sanitizer residual was at 100 ppm (minimal required: 50 ppm) upon two consecutive cycles at the plate level once the Manager connected the dishwasher to a new sanitizer pail, and primed the dishwasher. Manager informed dishwasher is checked twice daily (AM and PM checks) for sanitizer residual concentration at the utensil level. Chlorine test strips were available. Continue with monitoring the dishwasher final rinse chlorine sanitizer at the utensil level, and ensure the dishwasher dispenses at least 50 ppm chlorine sanitizer during the final rinse cycle to effectively sanitize the utensils/food contact surfaces.
