- 1. Green Room: Staff personal belongings (e.g., purse) were stored on a cabinet shelf accessible to children, posing a potential hazard if medications or other harmful items are present. The purse was relocated to a higher shelf during the inspection. Ensure staff store personal items in inaccessible areas.2. Red room: One of the electrical outlets were missing a cover.A cover was placed on the outlet during the inspection.

- 1. Yellow Room: The edge of the blue carpet is lifting, creating a tripping hazard. The tape securing the carpet is no longer sticky. Please resecure the carpet to ensure it remains flat.2. Green Room: The grass turf rug has a hole, and a section is damaged. Please trim the turf to remove the damaged section.3. Blue Room: The bleach sanitizer tested at 10 ppm, below the required minimum level. A new solution was prepared during the inspection and measured 100 ppm. Ensure sanitizer is maintained at proper concentration

Please find alternative playdough recipes.2. Blue room: Water beads observed in the room. Water beads as sensory play items should only be allowed for children over 5 years old under supervision.Please remove the water beads from the room.

- Alcohol based wipes were expired in 2017 inside one first aid kit.- Please replace the wipes.

- Front entrance door has small gaps along the bottom corner.- Ensure the door is sealed off and vermin proofed.

- White fridge storing kids lunches was measured at 10-13C throughout the fridge.- Ensure staff are monitoring the fridge temperature for storing kids lunches and maintain temperatures of <4C.- Staff removed all kids lunches to a separate fridge/freezer and photo of thermometer inside fridge showing <4C was received by PHI.
