      • *Two plates of food were being saved for residents—one covered with a napkin and the other with a plate cover. Both plates were found stored at room temperature on the dining room tables. Staff reported that the residents were still sleeping. It was reviewed with staff that food must not be held at room temperature, as this places it in the temperature danger zone. Food intended to be saved should be properly covered, refrigerated promptly, and reheated before service or placed into a hot holding unit. Corrected: Both plates of food were discarded on site.

      • *Recreational activity food items(e.g. yeast) was found in the servery cooler. Designate and label an area in cooler just for rec staff to prevent cross contamination.
    • 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
      • *Roll of paper towels was sitting on the counter. Place paper towels into a proper dispenser so it is easily accessible and to prevent contamination of the bulk roll.
    • 23. Is the facility maintained in a clean and sanitary condition?
      • *Cooler was dirty. Shelves were sticky and spills noted. Clean cooler and maintain sanitation. *Wooden storage unit used to hold servery items such as place mats, jams, peanut butter was dirty, stained, sticky and painted finish was chipping. Clean and refinish storage unit so it is smooth and easy to clean.
