- 03. Are clients at this facility being protected from hazards (Critical)?
- A shattered external glass windowpane was observed on the outside of the West pre-school playroom. While the internal glass windowpane was still intact, the openable part of the window below this windowpane may allow for broken glass to fall into the playroom. Please leave this window closed to prevent this hazard and repair the damaged windowpane as soon as possible.

- There are a few general maintenance and construction concerns, including baseboard trim issues in most playrooms, and missing or damaged ceiling tiles. Please make sure to repair all floors, ceilings, and walls so that they are smooth, easily cleanable, and in good condition.
- 10. Is a properly supplied first aid kit provided and readily available?
- While first aid kits were present, one was missing supplies (such as antiseptic wipes) and another kit contained expired antiseptic wipes. Please ensure that first aid kits are well-stocked and readily available.
- 18. Is the facility and equipment being maintained in a suitable manner (Non-Critical)?
- 1. Taped carpet observed in the OSC/preschool unit- new flooring needs to be durable and easy to keep clean. Program Director indicated the carpet will be removed and flooring will be installed.2. Ensure to tighten the screws on one of the cribs in the East infant room, as well as the wooden play structure in the West pre-school room to prevent any potential hazards from fallen screws.

- A daily and weekly toy and surface cleaning schedule/checklist was only available for the OSC room.Please have a comprehensive and specific daily, weekly, and monthly schedule/checklist for all playrooms rooms made available to staff to ensure that toys and surfaces are being cleaned at an appropriate frequency (e.g., mouthed toys are cleaned at least daily, all toys are cleaned at least weekly).

- Except the bathroom sink in the West pre-school room all other handwashing and diapering sinks were found to have water temperatures above 49.0 degrees Celsius (ranging from 52.8 degrees Celsius to 61.2 degrees Celsius, depending on the sink). Water temperatures above 49.0 degrees Celsius can cause severe scalding/burn injuries on children. Please install a mixing valve and/or temperature limiting device on each sink accessible to children so that water temperatures are below 49.0 degrees Celsius.

- Several mats on the West side appear torn on the surface, rendering them no longer in good repair, impervious to moisture, smooth, and easily cleanable. Please replace damaged mats.
- 25. Are there adequate storage areas sufficient for the operation of the facility?
- Toys and blankets were observed to be stored in multiple bathrooms, including in close proximity to toilets. Please ensure that stored toys are kept in covered bins and/or away from any potential sources of contamination.Children's water bottles were observed touching. Store them in larger bins or in a way where one child's water bottle isn't touching another child's water bottle.

- While steel wool has been installed in some wall gaps (particularly in the bathrooms), there are several areas that pests can infiltrate the facility from. An insect screen is missing on the openable West pre-school room window (the broken one), several areas in the bathrooms have gaps that are not fully sealed by steel wool, and ceiling tiles are missing and/or damaged in the East Infant room (near the kitchen door), and the East pre-school room bathroom. Please repair and/or seal all potential entry points for pests, and keep windows and doors closed when they are not being used for ventilation or the exit/entry of staff and children.

- 1. Two toilets were damaged and/or in disrepair. The toilet in West pre-school room bathroom had a tank lid that was repaired with sealant, but the sealant appeared to have visible contamination and was not easily cleanable. Please ensure that the surface of toilets are smooth and easily cleanable, such as by sanding down this grout/sealant.The second toilet (East pre-school room bathroom) appeared to be taped shut due to previous flooding issues mentioned by the operator. Please ensure that the toilet is repaired.2. The walls and baseboard areas of multiple bathrooms were in disrepair. Please make sure to fix these areas so that they are smooth, durable, easily cleanable, waterproof, and in good repair.3. The sink in the East pre-school room bathroom had grout/sealant that appeared to be covered in mold and/or other visible contamination. Please make sure that this sealant is smooth, easily cleanable, and continue to frequently clean and disinfect the bathrooms.

- 1. Multiple bathroom sink cabinets that contained chemicals such as chlorine-based sanitizer and/or laundry detergent were not locked (West pre-school room bathroom, East pre-school room bathroom, East infant room) and were at a height accessible to children. Please install child locks on these cabinets or remove any chemicals from areas accessible to children to prevent the risk of children accidentally consuming hazardous chemicals.2. A toilet plunger was found in a bathroom sink cabinet accessible to children (West pre-school room bathroom). Please remove the plunger or install child locks to prevent the risk of children accidentally coming into contact with fecal contamination.3. A shattered external glass windowpane was observed on the outside of the West pre-school playroom. - 1. While child gates were present at kitchen areas, they were observed to be open and accessible to children during the inspection. Please keep child gates to the kitchen area closed, especially when children are present in the playroom attached to kitchens.2. An electrical outlet at a child's height in the West OSC room was missing an outlet cover. Please install one to prevent any risk of electrical shock.

- 1. Chlorine-based sanitizer/disinfecting solutions tested at a diapering station was observed to be at a concentration below 100 ppm (East pre-school diapering area). This disinfectant solution is not effective at removing harmful bacteria and viruses when it is below 100 ppm. Please make new sanitizer solution daily, test it to make sure the concentration is correct, and use the sanitizer solution according to an approved diapering procedure. 2. Multiple diapering mats were found to be in disrepair and were no longer easily cleanable (East pre-school room diapering area, East infant room diapering area, East toddler room diapering area), while the underside of two diapering mats were found to have visible contamination present (East toddler room diapering area and East pre-school room diapering area). Please replace diapering mats that are not easily cleanable and clean and disinfect diapering mats properly with sanitizer solution according to an approved diapering procedure. 3. A written diapering procedure was not present at diapering stations. We recommend written diapering procedures be posted at all diapering stations, and a poster will be provided with this inspection report.

Please ensure that chlorine-based sanitizer is made freshly daily, tested with test strips to ensure the correct concentration, and is used frequently to clean and disinfect play equipment and surfaces.2. - 1. Staff personal items (water bottles, car keys, lighters) were observed to be stored alongside where food storage and other childcare activities take place (e.g., diapering). Please keep staff personal items separate from these areas to prevent any potential cross-contamination. 2. Toys and blankets were observed to be stored in multiple bathrooms, including in close proximity to toilets. Please ensure that stored toys are kept in covered bins and/or away from any potential sources of contamination.3. Diapers and babies' personal effects were observed to touch and were not individually labelled. Please label diapers and baby bottles, and keep them physically separate, such as in separate bins or cubbies.

- While steel wool has been installed in some wall gaps (particularly in the bathrooms), there are several areas that pests can infiltrate the facility from. The West OSC room door was left open (though it was closed at the time of inspection), an insect screen is missing on the openable West pre-school room window (the broken one), several areas in the bathrooms have gaps that are not fully sealed by steel wool, and ceiling tiles are missing and/or damaged in the East Infant room (near the kitchen door), and the East pre-school room bathroom. Food kept at this temperature can allow for the rapid growth of harmful bacteria and can cause illness. Please set the temperatures of all refrigeration units and coolers at the facility to <4 degrees Celsius and have temperature logs and thermometers available for each unit.
