      • Baby room2: the belted diaper pad was not attached to the counter surface The Velcro was not sufficient to keep the pad attached and therefore a belt must be affixed to the surface of the counter

      • CORRECTED DEC 13 1. Fabric couches were missing their washable covers throughout the facility.Ensure to have couch covers and some spare available at all times2. Deteriorated finishes around the hand dryer and sink area of the top floor Preschool room.Install a back splash to ensure the area is smooth durable and easy to clean.

      • 1. Baby Room Two: The bin with bleach solution disinfectant and paper towel was direct against the change pad.Cleaning supplies must be kept out of reach of children.Find an alternative storage spot for the disinfectant bottle

      • Baby Room2: there was a bin with baby bottles and water bottles direct against the diaper change pad. Find space away from the diaper area to store food related items. Diapering area is for diapering only
