- All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OBSERVED MOUSE DROPPINGS IN REAR STORAGE ALONG THE WALL, IN OFFICE UNDER DESK AND ALONG THE WALL, IN BASEMENT ON THE FLOOR AND UNDER SOME STOCK SHELVES. MUST CLEAN ALL AFFECTED AREAS AND CONSULT WITH PEST CONTROL OPERATOR. FRONT ENTRANCE DOOR IS NOT COMPLETELY RODENT PROOFED AT THE SIDE; MUST RODENT PROOF DOOR.

CLEAN INTERIORS OF COOLERS AND FREEZERS AND PREP TABLES IN REAR PREP AREA. ONE FREEZER UNIT IN REAR HAS A VERY RUSTY INTERIOR. UNIT IS NOT ON, INSTRUCTED TO CLEAN, PAINT AND/OR REMOVE FROM PREMISES.

- The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. CLEAN FLOORS IN AFFECTED AREAS MENTIONED IN VIOLATION #18 (UNDER SHELVES, BASEMENT, PREP AREA CORNERS, ETC).

- The walls and ceilings shall be in good repair and easily cleaned. OBSERVED SEVERAL DAMAGED CEILING TILE IN REAR NEAR REAR DOOR AND WASHROOM; MUST REPLACE. MUST CLEAN CEILING VENTS WHERE NEEDED TO REMOVE DUST NOTED. MUST REPLACE/PROVIDE MISSING WALL COVING AT WALLBASE IN OFFICE AND REAR STORAGE AREA. SEAL OTHER HOLES ALONG WALL AROUND PIPES WHERE NEEDED.

- Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. NO COLD RUNNING WATER AT EXPOSED HANDSINK IN REAR PREP AREA; MUST PROVIDE.
