    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • The lighting in room 3 and 4 are not working.Please ensure there is adequate lighting in room 3 and 4 and that all lights are working to ensure rooms can be properly cleaned.

    • 20. Is reusable equipment disinfected properly?
      • There is no high level disinfectant available to disinfect reusable metal tools (tweezers, extraction loops, etc.). The high level disinfectant on site is expired. Please replace the high level disinfectant with a new bottle if reusable metal tools will be used.
    • 25. Are the equipment manufacturer's instructions for use of specific equipment available and followed?
      • There were no manufacturer's instruction for use manuals or booklets available for the energy emitting devices throughout the facility (there are machines/devices located in each room 1-6).Please ensure manufacturer's instructions for use are onsite and available for each machine or device that will be used.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • There is no low level disinfectant available to disinfect general surfaces (beds, counters, tables, chairs, machines, trolleys, etc.). The preempt RTU disinfectant onsite is expired and alcohol cannot be used as a surface disinfectant. Please replace with a new low level disinfectant.
