      • Observation: Mouse droppings observed on the floor in the the following areas:
      • -Underneath shelving in the dry storage room (floor perimeters)
      • -On floor behind the plastic storage bins used for storage of bulk dry goods (oats, pasta, scone mix, muffin mix, etc.)
      • Corrective Action(s): *Safely remove rodent droppings (wet area with sanitizer/disinfectant and wear disposable gloves/mask when removing droppings), clean and sanitize affected surfaces.

      • Observation: Potential rodent attractants observed being stored directly outside the exterior entry doors to the kitchen (recycle bin, boxes of dry pasta, etc.)
      • Corrective Action(s): *Keep any potential pest attractants (food, garbage, etc.) stored away from the exterior doors to discourage rodents from congregating near the entrance.
      • *Ensure to keep doors tightly-closed as much as possible to block rodents from entering.

      • Observation (CORRECTED DURING INSPECTION): Used ice cream scoop stored in dipper well but tap for dipper well was turned off so water inside was stagnant.
      • Corrective Action(s): Ensure that the tap at the dipper well is kept on during each meal service when ice cream is on the menu.

      • Observation: Numerous mouse droppings found in the kitchen on the floor including under the dishwashing line, behind equipment in the grill line area (on both sides), in dry storage room and under baking station. Two areas observed where mice may be accessing the kitchen: gap in wall around pipe clean out on the wall under/left of the dishwasher and hole in white plastic drain cover behind large kettle. Pest control company in twice per month currently.
      • Corrective Action(s): - Clean up all mouse droppings in kitchen, ensure that all food is kept up off the ground and/or in durable pest proof containers.
      • - Notify pest control company of areas of concern, follow all their recommendations for remediating issue
      • - Seal up two potential entrance points noted above

    • 306 - Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation:
      • 1) Old mice droppings found along all the base of walls in the kitchen and food storage rooms, on lower shelves below the steam tables, and on some box/bags with food in them (in the dry storage room). Dropping contain bacteria which can get into food, and the scent of mice droppings and urine will attract more.
      • 2) There is mould and mildew on the ice shield inside the ice machine. The mould and mildew could fall/touch ice, which could make a person sick.
      • Corrective Action(s):
      • 1) Clean and sanitize all surfaces and areas that have droppings and urine on it. Throw away all foods which the box or bag (which the food is found in) have droppings or urine on it.

      • 2) Clean and sanitize the interior surfaces of the ice machine. Ensure to turn it off first, empty the ice out, and then clean & sanitize it.

      • Observation (CORRECTED DURING INSPECTION):
      • The lids, on the dry baking ingredients bins, are cracked and damaged. Pieces of plastic could fall into the foods and clients could consume them. This was mentioned in past inspection reports.

      • Purchase new lids and throw away the damaged ones.

      • Observation (CORRECTED DURING INSPECTION):
      • The plastic baffle, inside the ice machine has mildew/mold accumulated on it. The mildew/mold can fall into the ice and possibly make a person ill.
      • Corrective Action(s):
      • Empty the ice machine and wash and sanitize the baffle and all interior surfaces to remove and kill the mold/mildew. Ensure this is done on a regular basis. During the inspection, staff started the process of washing and sanitizing of the baffle.

    • 308 - Equipment/utensils/food contact surfaces are not in good working order [s. 16(b)]
      • Observation:
      • Damaged equipment and containers observed. This is unsafe as pieces of plastic could fall into food; the equipment can NOT be properly washed and sanitized; and it is creating an environment for pests to breed in. The following items were found damaged:
      • 1) Plastic food lids and containers were found to be cracked, chipped, and/or damaged.
      • 2) There is a leak in the ss sink basin prior to the dishwasher. There is an excessive amount of water pooling on the floor, and flies are harboring in it.
      • 3) The plastic guides on the dishwasher doors are cracked and broken. Water is spraying out and pooling on floor, and flies are harboring in it.
      • Corrective Action(s):
      • 1) Immediately discard all damaged equipment, lids & containers, and purchase new ones.

      • 2) Repair or replace the ss sink basin so it no longer leaks.

      • Observation: Observation: QUATS sanitizer bucket was at 100ppm and 200ppm is required to properly wash and sanitizer food contact surfaces. Concentrations lower than 200ppm will not effectively remove disease-causing pathogens.

      • Observation: Observation: The handwashing station in the corner by the walk-in freezer didi not have a liquid soap dispenser. Operator said that they do not use this handsink. However, the kitchen was approved according to having this handsink and as such, it must be stocked so that employees can wash their hands properly and frequently.
      • Correction: Install the liquid soap dispenser on the wall and ensure that soap and paper towels are always available.

      • Observation: Observation: Foods in the dry storage room and walk-in cooler were observed uncovered (e.g. no lid or lid not properly on) and therefore not protected from contamination.

      • Observation: Observation: The kitchen back door was propped open despite a sign saying that it must always be kept closed. the door will serve as an entrance for pests (this includes flies and rodents) that can then contaminate foods with disease-causing pathogens.

      • Correction: The kitchen back door must remain closed at all times. If air flow is desired, a screen that is securely attached to the door frame may be installed.

      • Observation: Observation: Various areas of the kitchen were found to be in unsanitary condition, showing debris:
      • 1) The line cook side of the kitchen: underneath stoves, line cook cooler, the server cooler and the ice cream freezer, the exhaust hood --> dust
      • 2) Dust on the underside hinge of the ice machine. These areas will attract pests and/or contaminate foods.

      • Observation: Observation: There were plastic lids used in the baking station on dry ingredients that were cracked (flour, sugar, etc) This is unsafe and unsanitary as pieces of plastic could fall in food and the damaged items cannot be cleaned properly (food accumulates in the cracks). This is a repeat infraction.

      • Observation (CORRECTED DURING INSPECTION):
      • Temperatures for the cook, cool, and reheating steps were not being recorded. This needs to be done to prevent growth of pathogens.
      • Corrective Action(s):
      • Adjust the HACCP/FoodSafety Plans so proper recording methods are utilized to ensure the cooking, cooling and reheating temperatures are recorded by staff. Retrain staff on the changes to these processes. During the inspection, the manager made changes to the recording sheet and will be retraining all staff on it.

      • Observation:
      • The plastic lids on the dry goods containers for baking ingredients (flour, sugar, etc.) were cracked, and found cracked & damaged plastic food containers & utensils in use. This is unsafe and unsanitary as pieces of plastic could fall in food and the damaged items can't be cleaned properly (food accumulates in the cracks).
      • Corrective Action(s):
      • Discard all damaged, chipped, and cracked food container, lid and utensils. Purchase new ones and ensure to conduct regular inspections of such items to remove the damaged ones. During the inspection, the manager removed the damaged food containers and utensils from use.
