      • The laser unit does not have manufacturer's instructions and furthermore the owner was told that the unit cannot be registered as it does not have a model number. Please discontinue the use of this laser at this time and confirm this to EPH in writing.

      • Last Inspection:- Cleansing of the skin is not being done prior to a service that may involve contact with mucous membrane, or that will puncture or may potentially break skin. Current Inspection:- Cleansing of the skin is not being done prior to a service that may involve contact with mucous membrane, or that will puncture or may potentially break skin. Ensure cleansing is done prior to waxing.

      • Last Inspection:-Equipment used to puncture the skin or mucous membrane was not being discarded after use on one use. Current Inspection:-Equipment used to puncture the skin or mucous membrane was not being discarded after use on one use. Metal lancets were observed on-site but the operator indicated that extractions are not done. If this is the case please remove.

      • - Used semi-critical and critical equipment was not being adequately cleaned and dried prior to disinfection. Ensure equipment is: disassembled, cleaned to remove soil, and rinsed and dried prior to disinfection or sterilization.

      • Last Inspection:-The laser unit was visibly dusty. Please ensure that it is wiped down after each use.Current Inspection:- The laser unit was noted as visibly dusty.

      • Last Inspection:- Personal services workers are not following all of the disinfectant (Pre-empt CS20) manufacturer's instructions for use. - High level disinfectants are not being tested and verified daily using proper test strips. - Disinfection records were not available. Current Inspection:-Operator was using water to clean tools and then spraying with Pre-empt CS20. This is not in accordance with manufacturer’s instructions. As all re-useable tools at this facility are non-critical, a low or an intermediate level disinfectant can be used. Please clean tools with soap and then use an appropriate disinfectant in accordance with manufacturer’s instructions.
    • 24. Are waste sharps disposed of properly?
      • -There was no suitable puncture resistant container available for the disposal of sharps. Please ensure an appropriate sharps container is available.

      • - The personal services facility is not maintained in a visibly clean state. Ensure additional cleaning is done.

      • -The following concerns were noted with the laser device* There was a mirror in the room. Please ensure there are no reflective surfaces in the same room as the laser.*There was no warning signage on the service door. Please post appropriate warning signage for a radiation emitting device.*There was no manual available. Please ensure there is a manual available.*The laser device is not registered. Please ensure the laser device is registered.*The cords coming from the laser had covers that were disintegrating, making them difficult to clean. Please ensure that the cords are repaired and maintained in an easy to clean condition.*The laser device was visibly dusty. Please ensure that it is maintained in a clean condition.
