      • Observation: Minor mouse activity noted in storage room. Likely in other areas as well. Management working with pest control to block more access points. Foam insulation bits observed in gabage cubby on cookline and may not be the best way to block access. Fixed wired mesh or steel wool combination may be more effective at controlling access, especially around larger conduit openings or along extended gaps. Discuss with pest control.
      • Corrective Action(s): Continue to work with pest control to strategize access control, keeping in mind cleaning and sanitation practices will also help control pest populations. Population control through trapping will help as long as traps don't get disturbed or get put back when moved for cleaning.

      • Observation: Some areas of the kitchen could be cleaner. Notable areas include as discussed: Bar floor behind and by deep freeze, and beverage cooler at syrups; cookline at garbage cubby on pass through side, and along edge of floor below new sushi station; behind the rational oven and to a lesser extent the corners of the dish pit.
      • Corrective Action(s): Make sure staff get to these spaces on a more frequent basis or in a more thorough manner.

      • Observation: Mouse and fruit fly activity noted in facility. Mouse droppings observed in some corners and nooks and hard to reach places. Droppings most noticeable in dry food storage room in back corner, including dropping on lowest shelf. Dishpit where baseboard/wall broken also noteable. Many snap traps in facility, and many of those not set, or upsidedown or not properly placed.
      • Fruit flies noted in corner of dishpit under counter. Unsure why this space may be problematic, but the mouse traps in this space or something inside wall at plumbing exit may be contributing to populations. Also noted in bar area and janitor space.
      • Corrective Action(s): Discuss mouse trap placement and strategy with pest control technician. Continue to seek out movement patterns and place controls as discussed.
      • Fruit fly control dependent on cleaning up liquid sugars, stagnant water ponding areas, and places where biomats can flourish (i.e. Floor drains, dishpit and janitor areas in general and bar). Target these environments on daily basis to break the breeding cycle.

      • Observation: Drain/fruit fly population noted in dish pit and mop/chemical areas of kitchen. Minimal overlap into food prep areas. Significant reduction from previous inspections.
      • NOTE: Mop sink area much drier than normal, and is reflected in less flies noted in this area. Be aware of dust pans (stored above mop sink). they are dirty and likely contributing as food source and to breeding.
      • Corrective Action(s): Continue to apply daily clean up controls to this area (and dish pit) to further reduce breeding possibilities for drain/fruit flies as discussed. Daily controls for a couple weeks now may help peak season summer fly populations.

      • Observation: Overall facility cleaner and better organized than observed in past inspections. Good.
      • Observed areas of the kitchen that could use more cleaning attention include:
      • - the floor of dish pit in the corners and around chemical buckets;
      • - Dust pans at mop sink area;
      • *Bar area shelving as discussed.
      • Corrective Action(s): Target above noted areas as discussed. Use chemicals (floor cleaner, sanitizer, detergents etc.) as added fly breeding deterent in the drains, dust pans and general wet areas of kitchen and dishpit as discussed.

      • Observation: Improvements to fruit/drain fly population observed. Still notable in mop sink area, dishpit and to lesser degree walls in secondary areas. Pest control provided pheromone traps, but no other advice. Daily quality cleaning to key breeding environments will be necessary to eliminate problem.
      • Corrective Action(s): Continue to target key breeding areas as discussed. Find appropriate chemical (possibly foam spray) that can be applied to and act on difficult to access breeding environments like drains, dishwasher filter basket, underside of dishpit work counters, and floor grout where water tends to sit. Recommend to make this chemical spray application part of the last step after end-of-day cleaning.
      • Review at next scheduled inspection after renovation work in August.

      • Observation: Floor grout in kitchen looks better, but is still subject to accumulations of food debris. Daily cleaning process includes brushing floor and wet vaccuum clean up.
      • Corrective Action(s): Continue to target kitchen and dishpit.mop sink areas for intensive daily cleaning.

      • Observation (CORRECTED DURING INSPECTION): Bottom left drawer of far cookline cooler observed and measured consitently at 7 C.
      • Insert cooler at sandwich station measured 10 C.
      • Corrective Action(s): Keep potentially hazardous foods to max 2 hours and discard, or supplement with ice or relocate to unit that can maintain 4C or less on more consistent basis.
      • Have these units serviced such that they can maintain max cool.
      • **kitchen is very hot and will impact cooling units. Air conditioning planned for cookline.

      • Observation: Major fruit/drain fly infestation in kitchen, most notable in the mop/chemical station and dishpit. Hundreds of flies observed on walls in both areas. Due to poor lighting conditions in kitchen cookline, difficult to assess extent of fly activity in this area. Pictures taken.
      • Corrective Action(s): Contact pest control and develop a comprehensive action plan to reduce and eliminate drain/fruit fly population on both an immediate and long-term time frame.
      • Review at re-inspection date. Provide updates to this office when available.

      • 305 - Conditions observed that may allow entrance/harbouring/breeding of pests [s. 26(b),(c)]
      • Observation: Multiple areas of kitchen environment observed contributing to fruit fly drain fly breeding. These include:
      • A) Leaking drain pipes below sink fixtures with significant accumulations of bioslimes along the outside of these pipes and open buckets to catch the leaking water.
      • B) Accumulations of stagnant waste water falling onto a multitude of surfaces that are not regularly or adequately cleaned.
      • C) Dishwasher waste filter basket and reservoir subject to accumulations of food debris and resultant biomat.
      • D) Floor cleaning equipment (mop, buckets, shop vac, broom and dustpan etc.).
      • E) Mop sink and other drains.
      • F) Floor grout lines in all areas of kitchen. They have become grooves of significant food debris and in areas moist nutrient enriched "gleyed" biomat.
      • Corrective Action(s): Complete and comprehensive clean up of moisture laiden areas and removal of food debris accumulations required as per existing corporate sanitation plan. Revise sanitation plan to include daily clean-up of key breeding environments as discussed. Provide update to office. Will review at follow up inspection.

      • Observation: A) Lighting in the kitchen (cookline) area is inadequate for staff to properly perform functions.
      • B) Use of squeeze bottles for handwshing stations along cookline not adequate.
      • C) Floor grout lines throughout kitchen area are quite deep proving difficult to clean. This has resulted in significant accumulations of food debris and stagnant water in these areas.
      • Corrective Action(s): A) Add lighting in the servery/cookline area such that it conforms to the Canadian Food Code. Submit a proposed timeline for correction. To be reviewed at follow up inspection date noted.
      • B) Install wall mounted soap dispenser for both of these handwash stations.
      • C) Regrout the entire kitchen in a manner that promotes easy and complete cleaning.
      • Submit timeline for all of the above prior to follow up inspection date noted.

      • 2) Countertop milk fridge at 8 or 9 C. Significant frost build up noted.
      • Corrective Action(s): 1) Rechecked unit after adjustment. Temperatures again under 4 C.
      • 2) Take this fridge out of use, move product to other cooler or fridge and defrost unit. Done.

      • Observation: 1) Food product carts observed mostly dirty to very dirty.
      • 2) Shelving/floor in walk-in at sauces observed dirty. Discussed with Chef.
      • **Rest of unit, including keg section pretty good.
      • 3) Floor drain below prep sink observed very dirty. Discussed with Chef.
      • Corrective Action(s): Clean these noted areas as discussed. Make sure to incorporate the Shop Vac for extraction prior to any cleaning and treating.
      • **Recommend recycling bottle bin get sprayed out EACH time it is emptied, and perhaps shot with Wash n walk to keep an inhospitable environment for fruit flies. Take extra care as you move into fly breeding season.

      • Corrective Action(s): This unit must do better job of keeping foods below 4 C. Defrost unit. Place thermometer in unit and monitor. Re-shuffle food storage in servery and/or replace unit as necessary.

      • - Bar slushy machine (minor);
      • - Dry food storage bins (minor) (wheeled);
      • - Rear prep station - floor/corners;
      • - Walk-in cooler floor (minor);
      • - Walk-in keg cooler floor (**increase lighting - replace bulb);
      • - Walk-in Freezer - Defrost, clean up - audit condenser line for blockage and/or install door baffle to reduce ambient air flow into unit.
      • Corrective Action(s): Clean up and address other issues by follow up inspection date noted on report.

      • Observation: Chemical storage area continues to leak water and/or chemicals onto floor. Chemical odour evident in immediate area. Slimy matt accumulating in spots beneath containers and along floor wall interface.
      • Corrective Action(s): Improve daily/or more clean up and maintenance of this area to eliminate or substantially reduce ponding water and slime build up.
      • **Do not place or store brooms/mops on floor in this area. Find alternate location for broom storage. Water control must improve in this area. Consider relocating all containers off floor in area to allow easier cleaning by staff.

      • Observation (CORRECTED DURING INSPECTION): Dishwasher measured 0 ppm using supplied test strips. Observed chlorine line empty and not primed. *Possible wonky feeder spring at supply-end of system. Primed system and rechecked with operators' strips. Again 0 ppm. Re-checked with my strips and measured 50 to 100 ppm.
      • NOTE:** Rinse temperature checked with IR gun and measured 178 F. Double sanitizing?
      • Corrective Action(s): Ensure chlorine feed is primed at beginning and midpoint of day to ensure unit is ready to sanitize and that it keeps sanitizing throughout day. Replace strips with new low concentration chlorine strips.

      • Corrective Action(s): Extend drain cleaning as discussed to include drain by walk-in cooler and mop sink area/drain.
      • ***Mop sink area very wet with water ponding in corners and around area in general. Recommend ensuring a mop bucket remain below dispensing hoses to minimize drip effect.

      • Observation: Several areas of the kitchen and storage areas observed not adequately cleaned. These include:
      • - Two line coolers for sauces (internal cavity area and shelves);

      • - Floor corners in line kitchen;
      • - Edges and corners of prep and work surfaces in line kitchen;
      • - Walk-in cooler and rear prep area floor (under shelves, around legs);
      • Corrective Action(s): These areas need additional cleaning (frequency and/or quality) as they are spreading into direct food prep stations. Complete follow-up inspection date noted on report.
