    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • 1) A few droppings were observed along the windowsill in the back dry storage room as well as on the floor in the back hallway/entryway. Please safely clean up the droppings and sanitize the affected areas. 2) Holes were observed by the back main floor exterior door which could be acting as entry points for mice. Please seal to prevent entry.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Along the back of ice machine, a greasy substance appears to be leaking from the vent. Please clean and ensure the ice machine is operating properly.

    • 08. Do food handlers ensure that food is protected from contamination (unless noted elsewhere)(Non-Critical)?
      • 1) Bulk dry ingredient bags that are opened should be stored in sealed containers. 2) As well deli containers were being used as bulk scoops for dry ingredients. Please switch to scoops with handles to protect from contamination.3) Knives were being stored between the pizza oven and a prep table. Please store knives in an alternate location as this is not a clean or sanitary manner of knife storage.
    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • 1) A dead mouse was observed in a snap trap in the back hallway leading to the exterior exit. 2) In the dry storage area droppings were seen in each corner and along the windowsill and radiator. What appeared to be droppings were on the baking pan with immersion blender (the pan and blender were taken for cleaning and sanitizing at the time of the inspection). Droppings were also observed along the back hallway near empty bottle storage (please rinse all bottles after use), and under the dishwasher. Please safely clean up all droppings and sanitize all affected areas. 3) Multiple tin cat traps were out of place. Please ensure staff make sure they are oriented with hole along the wall after cleaning.4) Holes were observed by the back main floor exterior door which could be acting as entry points for mice. Please seal to prevent entry.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • A broken tile in the wall behind the rack of equipment that is nearest to the prep sinks was observed. Please repair the hole and finish such that the walls are smooth, cleanable, and impervious to moisture.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Along the back of ice machine, a greasy substance appears to be leaking from the vent. Please clean and ensure the ice machine is operating properly.

      • Metal wire sieves stored above the two-compartment prep sink were broken and has loose wires on them. Please discard all broken or damaged sieves to reduce the risk of physical contamination.

      • Hard to reach areas under and behind cooking line prep coolers have an accumulation of grime and debris. Please deep clean the facility.

    • 04. Does the operator use approved methods for cleaning and sanitizing equipment, utensils, and surfaces?
      • Multiple quat sanitizer bottles were tested and 0ppm was detected. When tested at the dispenser 0ppm was also detected. Please manually mix the sanitizer to 200ppm until the dispenser can be repaired. Please also ensure that staff are testing the sanitizer solutions daily to ensure it is dispensing properly (200ppm).

      • There were no test strips available on site for the QUAT sanitizer. - Please acquire appropriate test strips to test the concentration of the surface sanitizer.
    • 21. Is the facility properly constructed and maintained in a state of good repair to allow for the safe and sanitary handling of food?
      • 1) There were two broken light covers in the dishwashing area. - Repair or replace the light covers. 2) There were holes in the ceiling in the main kitchen area. - Please repair the holes and ensure the ceiling is a smooth surface that can be easily cleaned.
