      • Written procedures were not in place for the cleansing of skin and antiseptic procedure. Cleansing of skin procedure was completed during inspection. *Write a procedure for applying an antiseptic product where a critical instrument is used.
    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • Alcohol-based hand rub is expired. *Obtain an alcohol-based hand rub with a DIN or NPN number that is not expired.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • An antiseptic product containing a DIN or NPN is not available for the skin cleansing and antiseptic step. *Obtain an antiseptic suitable for the intended skin (lips, eyebrows, and eyelids) that is required to be prepped to inactivate, reduce, prevent, or arrest the growth of microorganisms.
    • 20. Is reusable equipment disinfected properly?
      • Test strips are not available to verify a pass / fail for the high level disinfectant that is re-used. *Obtain disinfectant test strips to monitor the pass / fail for every day that the disinfectant is re-used.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • Certificate of sterilization is not available. *Obtain a certificate of sterilization or proof of sterility on sterile products from the manufacturer.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Carpeted flooring is not cleanable. *Ensure personal service work area contains cleanable flooring.
    • 31. Are sinks accessible and used appropriately for the personal services provided?
      • Mobile hand sink with foot pump and cold water is not acceptable. Hot water is not available at the sink. *Use bathroom sink for hand washing, followed by a second hand-hygiene step using alcohol-based hand rub to complete hand hygiene. *Use facility reprocessing area for reprocessing tweezers and scissors used for eyelash extensions.
