- Observation (CORRECTED DURING INSPECTION): One shelf had a few old mouse droppings on it where two pieces of food contact equipment were stored.
- Corrective Action(s): Two pieces of equipment in question were removed during inspection and recleaned and sanitized. Ensure that especially low solid shelves (as opposed to wire shelves) are checked and cleaned regularly.

- Observation: Two hand washing stations found with the wall mounted soap dispensers broken. There were spray bottles with hand soap located at each of the hand washing stations however these are not as easy for staff to use.

- Observation (CORRECTED DURING INSPECTION): Hand wash station near juice dispensers lacked soap. Staff stated that person who refills all dispensers for facility is back from vacation today and is on his way.

- Observation:
- Found mice droppings along the base of walls, under platforms and on lower shelving units. Dead mice found in the metal tin traps. This is a clear indication that there is a large mice infestation.
- Corrective Action(s):
- Eliminate the mice infestation by:
- 1) Increase the frequency at which the professional pest control company monitors and services the premises. Trapped mice shall be removed daily by the pest control company.
- 2) Seal all openings in walls, floors and ceilings around pipes, wires, and duct vents with metal mesh (openings 1/4" or smaller) or metal covers. Remove the aluminum ducting tape, as this will NOT stop or prevent mice entry and it is not durable to withstand daily cleaning and abuse.
- 3) Install floor drain covers to all floor drains. Ensure the openings are not bigger than 1/4" Replace any damaged or missing floor drain covers.
- 4) Firmly stuff all small openings or cracks with steel wool and then seal with expanding foam.
- 5) Follow and complete all recommendations made by the professional pest technicians in their reports and/or in their discussions with you.
- 6) An inspection of the full facility (interior and exterior) should occur by the pest company, to determine other entry points and harboring areas. Treatment to all active mice infestation areas shall occur.

- Observation:
- There is a space at the base of the door in the dishwashing area, leading into the hallway. Pests, entering from other areas of the building can access the kitchen area this way.

- Install a door sweep or door guard at the base of the door to prevent the entry of pests into the kitchen area.

- Found moderate amounts of rodent droppings along the base of all walls, under platforms & tables, and by drain pipes. This is even with staff cleaning at the end of the night, and in the morning again. The droppings and urine have bacteria and viruses in it, and the oil from the mice act as an attractant or tell other mice the safe areas to navigate.

- Increase the frequency detailed cleaning is done to a weekly basis to remove the droppings, urine and mice oil deposits. The use of hot water, soap and a steam cleaner will greatly help in keeping the kitchen environment clean, and discouraging the mice from coming around.

- Observation (CORRECTED DURING INSPECTION):
- 1) Found frozen, raw chicken breasts thawing in the walk-in-cooler, on a shelf above RTE foods. Raw chicken juices could drip on foods and contaminate it with bactera.
- 2) Found containers/packaged of food (rice cakes, sugar, salt), in the dry storage area, not covered. The food could become contaminated with dust or bacteria.
- Corrective Action(s):
- 1) Relocate the thawing raw chicken to the lowest shelf, or on a separate shelf away from RTE foods. During the inspection, staff moved the raw chicken to the lowest shelf in the cooler.
- 2) Place lids on the food containers and keep them on, until food is needed. Replace the lid immediately one the food is obtained. Wrap the open packages of food with saran wrap or place in a covered food container. During the inspection, lids were placed on the food conatiners, and staff were wrapping the opened rice cake packages with saran wrap.

- Found cobwebs around the legs of work tables, across from the steam kettles. This is an indication that cleaning of these areas hasn't been done in a long time.

- Observation (CORRECTED DURING INSPECTION):
- 1) Expired tofu was found (Mar. 15/19) in the walk in cooler. The product is unsafe to consume.
- 2) Staff are storing personal items (coats, bags, etc) with and/or beside food items in the dry storage/staff room. This practice has been mentioned in past inspections and this practice could contaminate the food.
- 3) Found uncovered containers of food (sugar, etc.) in the dry storage room. The foods can be contaminated.
- Corrective Action(s):
- 1) Immediately discard the tofu. During the inspection, the tofu was thrown away.
- 2) Inform staff to store their personal belongings in the designated areas, away and separate from food. It is recommended that small lockers or cubbies be stalled to encourage this behavior.

- Wash these areas with hot water & soap, and sanitize. More frequent, detailed cleaning is needed to address these problem areas.

- 2) The drain pipe under the 2-comp sink, in the drink jug washing and filling area, is leaking. Water is pooling on the floor which will harbor pests.

- 1) Immediately throw away any damaged food bins and lids, purchase new ones. Ensure to inspect equipment, bins, and utensils on a daily basis for damage.

- The exhaust canopy has accumulated food stains and dust. This will attract and harbor pests. The service sticker on the canopy indicates the next cleaning, by a professional company, should of been done in June 2017.

- Observation:
- Observed a large population of drain flies in the mop bucket, dishwashing, and jug washing areas. This was mentioned in the previous inspection, and a fly light was installed in the jug washing area but this doesn't seem to be effective. The presence of food debris and pooling water on floors, drains and equipment will harbor these pests. The odor of decomposting food by floor drains was detected.
- Corrective Action(s):
- The floor drains in the premises (especially in the mentioned areas) shall be professionally cleaned and treated for drain flies on a regular basis by a professional cleaning/pest control company. Fax or email the invoice to our office ([email protected]) as proof that this has been completed.

- 1) The ceiling tiles have accumulated dust on them. The dust can fall into food and contaminate it.

- 3) The exhaust canopy has a service sticker on it indicating the next cleaning to be completed by June 2017. Accumulation of food and grease in the exhaust hood, filters and vents is a fire hazard.

- 1) The upright cooler storing bagged lunches was leaking water from the compressor area. The water was dripping from the top of the unit, down the door, and pooling on the floor. The pooling water is a slip hazard and will attract pests.
- 2) There were some plastic food bins and lids that were cracked and/or chipped. Damaged food containers, equipment or utensils could allow pieces of plastic to fall into food. As well, damaged food equipment can NOT be properly cleaned (bacteria hides in the cracks).

- 1) Have the cooler unit maintenanced so that water no longer leaks from the unit and it works as per manufacturer's specifications.
