      • Observation: Floor of cook line, especially under grill is VERY dirty. Showed and discussed with mgt.
      • Corrective Action(s): This needs to be cleaned on daily basis, and verified by mgt and supervisory staff. Discuss roles and responsibilities with kitchen staff and cleaners to ensure job is completed. This will be reviewed on follow up inspection date noted.

      • Observation: Drain/fruit fly issue continues to be a significant problem. Has gotten worse since initial inspection when they were observed and appears out of control. Observed in the following areas of premises:
      • - Staff change room, especially in "out of order" toilet room - extreme (picture taken);
      • - Bar area, especially at far end - extreme (pictures taken);
      • - Dish pit - moderate;
      • - Chemical and mop sink room - significant;
      • Also noted in secondary areas including food prep station in back of premises.
      • **Reviewed Pest Control invoice. No recommendations noted. Only inventory of traps deployed/assessed.
      • Corrective Action(s): Obtain set of comprehensive recommendations from Pest Control to substantially reduce fly population in premises as noted above. Develop a comprehensive plan in consultation with pest control, including "next level" interventions.
      • See attached Correction Order for detailed timelines for correction.

      • Observation: Observed the following areas in an unsanitary condition:
      • - Under the grill line in main kitchen area;
      • - Dish pit area;
      • - behind ice machine;
      • - shelves and beverage dispensing units (discussed and identified to bar manager);
      • - floor drains in association with glasswashers.
      • Corrective Action(s): Clean the kitchen floor under the cook line. These pieces of equipment are on wheels and should be moved regularly as part of the cleaning process.
      • In an effort to improve the fly infestation, all above identified areas should be targeted for complete cleaning and secondary areas also should be reviewed and cleaned up as necessary. **Seek Pest Control for advice in this area.

      • Observation: Fruit fly infestation persists to significant degree in the bar area and employee washroom. Dish pit area much improved and fly populations at low to moderate level.
      • Corrective Action(s): Bar area - more commitment to fruit fly elimination needed. Removal of "dead" beverage line not completed and must be done; Any standing water and accumulation of biomat must be eliminated daily.
      • Employee washroom - investigate and eliminate ongoing water leak at water closet/toilet; investigate and eliminate water inside walls in this area.
      • Dish pit - target seams and joints on the underside of the disher station by daily cleaning and aggressive cleaning and disinfection as discussed.

      • Observation: Fruit fly infestation remains in bar area and has become much more significant since inspection dated May 31, 2018. Significant flies observed ontop of the bar counter, at all of the beverage/mix dispensers stations including the sinks, in the nozzle of these dispensers not currently immersed in water, under counters in severarl stations, at and along plumbing work, and generally flying around. In discussion with staff flies are migrating into the kitchen area of store. Existing traps set by PCO observed empty. Pictures taken of some of the more significant harbourage areas.
      • Corrective Action(s): See attached Correction Order to eliminate or substantially reduce fruit fly infestation by date noted on Order.

      • 306 - Food premises not maintained in a sanitary condition [s. 17(1)]
      • Observation: - Some parts of the bar are not sufficiently cleaned to help mitigate the fruit fly infestation as discussed during previous inspection and directed by Pest Control technician at the time. Water and/or beverage continues to accumulate and stagnate on many shelves, in corners, on floor, at and around glasswasher, along and under the extensive matrix of exposed plumbing, and in hard to reach areas of bar.
      • - Some parts of the bar design, primarily of wood construction already beginning to show signs of rot and deterioration and is likely affecting the cleaning process and subsequently the breeding opportunities for the flies.
      • - Beverage and waste transport lines observed slimy and accumulations of organic matter inside and around these lines remain unchanged from previous inspection.
      • Corrective Action(s): Improvements to overall cleaning are required to impact fruit fly infestation. Bar redesign may be required should sanitation not be significantly improved. This will be reviewed upon follow up inspection date noted on Correction Order.

      • Observation: Significant fruit fly infestation persists. Observed adult fruit flies on a multitude of surfaces and in abundance under counters and around beverage dispensing units. Observed a multitude of breeding habitat (as identified by pest control operator) throughout the bar area. Kitchen area not reviewed as it appears the bar area is the primary breeding ground for the fruit flies.
      • Corrective Action(s): Implement cleaning procedures as highlighted by Pest Control Operator (PCO). Implement remediation to bar area as highlighted by PCO items noted in the public comments section of this report. Follow schedules identified in PCO report/proposal and in this report to reduce adult fly population, fly breeding habitat and bar design deficiencies which could contribute to present or future breeding potential for fruit flies. Discard any product that becomes contaminated by fruit flies.

      • Observation: Drain flies continue to be a problem. Operator still waiting for instruction from Pest Control. Drain flies observed around bar drains and area. Glasswasher drip tray and drain is not designed properly to allow ALL water to easily drain. Observed almost an inch of water in tray and a selection of lemons/limes. This is a drain fly/fruit fly attraction. Discussed with operator - this is a known problem and servicing is planned.
      • Corrective Action(s): A) Reveiw drain fly remedies with Pest Control and implement changes. Supply list of corrective actions related to this prior to re-inspection date or have written plan to review on follow-up inspection date.
      • B) Drip tray to glass washer must be cleaned daily until permanent solution found.
      • C) A redesign of the glass washer drip tray is required such that water drains freely into drain.
      • D) A maintenance plan is needed for all bar drains to help control fly problem.
      • A to D will be reviewed at follow up inspection date noted on report.

      • Observation: Moderate drain fly population noted, mostly in the front bar area, but also in the kitchen and storage area of premises.
      • Corrective Action(s): A) Contact pest control technician to strategize control and elimination plan.
      • B) Improve cleaning in bar area - remove all construction sawdust; clean up floor and eliminate or control ponding water and residual moisture build up to better manage breeding areas.
      • C) Existing drains must have their own "improve" maintenance schedule to help break breeding cycle.
      • This will be reviewed at next scheduled inspection.

      • Observation: Dishpit observed not adequately maintained. Dish drip shelf excessively dirty and organic matt noted developing at edges of shelf. The dishwasher cavity also noted with significant mineral and grease build-up.
      • Corrective Action(s): Properly clean the dishpit focussing extra cleaning to above mentioned areas. Adjust written cleaning procedures such that staff better clean and maintain this station. Complete this cleaning by follow up inspection date noted on report.

      • Observation: No drain mats observed on trays used to dry glassses at bar. Glasses (shot glasses etc.) stuck to trays. Also observed the area where drain mats provided (by glasswasher) no containment is provided, which could result in water dripping onto floor.
      • Corrective Action(s): Make sure all glasswashing includes the use of drain mats and the drain mats are contained properly. Alternatively, ensure drainage occurs only into proper sink/drain. This will be reviewed at follow up inspection date noted on report.
