- Observation: Lighting fixtures made of glass were uncovered at the entrance of the walk-in cooler. - Observation: Lighting fixtures made of glass were uncovered at the entrance of the walk-in cooler. - Observation: Ice machine appears to be dirt build-up and slimy when swabbed with alcohol wipes. Public health significance: Biofilm build-up in ice machine may serve as breeding site to pathogens, which in turn contaminate ice cubes and can lead to water borne illness.
- Corrective Action: Operator to ensure ice machine is cleaned properly and regularly. Operator to ensure ice machine is swabbed with alcohol wipes from time to time to ensure it is clean and sanitary.

- Observation: (CORRECTED DURING INSPECTION) Wiping/cleaning cloths left on the counters and not submerged in a sanitizing solution between use. Public Health significance: Wiping cloths not submerged in a sanitizing solution may support the growth of pathogens and be a source of contamination to food contact surfaces, which could lead to foodborne illness.
- Corrective Action: Operator to ensure wiping clothsare fully submerged in a sanitizing solution of 100 ppm chlorine or 200 ppm QUATs at all times.

- Observation: (CORRECTED DURING INSPECTION) Sanitizer buckets and spray bottles not properly labelled. Public health significance: Unlabelled chemical containers in the food premises may lead to accidental use among staff members, which could lead to chemical contamination of food contact surfaces, use of the wrong sanitizing solution on food contact surfaces which may not support effective sanitizing of the surfaces and may lead to pathogen build-up.
- Corrective Action: Operator to ensure all sanitizer buckets and spray bottles are properly labelled.

- Observation: Several containers of food items were observed to be stored uncovered and on the floor of the walk-in cooler. (Public Health Significance) - Storing food items uncovered and on the floor can be easily accesed by pests that may disease causing organisms. Pests may contaminate the food items and cause foodborne illness. Morever, storing food items uncovered can cause physical contamination by dust, food debris.
- Corrective Action: All food items in the food premises should be covered and stored 15 cms off the floor and 5 cms off the walls.

- Observation: Ice machine had accumulation of slimy biofilm layer. (Public Health Significance) - Formation and accumulation of biofim layer on ice machine can harbour pathogens that may contaminate the ice beneath. Consumption of contaminated ice can cause foodborne illness.
- Corrective Action: Ensure that the ice machine is frequently cleaned and sanitized.

- Observation: Observed several items (fridges, tubs, chairs, sink, storage cabinets) outsite the kitchen backdoor. (Public Health Significance) - Area outside the kitchen backdoor should be kept clear and clean to prevent attraction of pests that may carry disease causing organisms. Pests may enter the enter the food premises, contaminate the food and cause food-borne illness.
- Corrective Action: Ensure that the items outside the kitchen backdoor are removed and the area is kept clear and clean.

- Observation: Some broken tiles, damaged shelving, and open portions of walls were observed during the inspection. Public Health Significance - improper maintenance of facility can make surfaces hard to clean, as well as provide food or areas of harbourage for pests.
- Corrective Action: Open portions of walls should be adequately repaired to prevent harbourage or entry of pests. Walls, flooring, and food contact surfaces in the facility should be in good repair and constructed of materials that are durable, easy to clean, and impervious to moisture.

- Observation: Observed lighting in cooler and mop sink area without covers during the inspection. Public Health Significance - Contamination of food with broken glass can cause serious injury.

- Observation: Due to facility being closed for approximately two years for renovations, the food premises must go through the permitting process again in order to operate. The facility was open to the public at the time of the inspection without completing this process.
- Corrective Action: Correct the critical violation listed in the inspection report by Jan. 13 to complete the permitting process and avoid enforcement action.

- Observation: (CORRECTED DURING INSPECTION) Garbage bag was used to cover dough. Public Health significance - Garbage bags are not food-grade, and can contaminate food with chemicals that can cause illness.
- Corrective Action: Food-grade materials, such as plastic wrap or clean cloth, should be used to cover dough while storing short-term.

- Observation: Inadequate sanitizer concentration detected in some sanitizer containers and spray bottles, as well as the glassware dishwasher. Public Health Significance - sanitizing solutions must be strong enough to destroy bacteria on surfaces to prevent the contamination of staff, clients, or food in contact with the improperly sanitized surface.
- Corrective Action: Change sanitizing solutions as required to always maintain an effective concentration (100 ppm chlorine or 200 ppm QUATs for surfaces, 50-100 ppm chlorine for glassware/dishwashers) and follow manufacturer's instructions for preparation and contact time. Keep test strips on-site to ensure that sanitizers are the correct concentration before use.

- Observation: Some hard-to-reach areas, such as behind freezers or underneath equipment was noticeably dirty. Public Health Significance - Inadequate cleaning can provide food for pests and lead to infestations.

- Observation: Thermometer in walk-in cooler was reading -6 degrees Celsius, whereas a calibrated infrared thermometer was reading between 0.5 to 3 degrees Celsius.
