      • - Sliding door leading into the reprocessing room was in disrepair. The door was stuck halfway open and hindered access to the room.- The laminate countertop underneath the autoclave as peeling.Corrective actions:- Repair, replace, or remove the sliding door leading into the reprocessing room.- Repair or replace the peeling laminate countertop underneath the autoclave.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Written procedures for skin cleansing, antiseptic, use, and post-service care instructions were not found during the inspection.
    • 21. Is equipment that is intended to be sterile maintained sterile at the point of use?
      • Several packages of sterile equipment such as forceps, openers, and clamps were visibly stained, or broken. These packages were no longer sterile at the point of use.Corrective actions: Please ensure all affected packages are re-packaged, and re-sterilized, after a satisfactory BI result. Should packages continue to become damaged, consider placing tip protectors on sharp edges of piercing tools before they are packaged.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • - Last completed BI was April 18, 2025, however a cycle was completed on May 21, 2025.- The results of the BI were not documented in the sterilization log- Type 5 CI were expired since 2023-01-06.Corrective actions:- Immediately acquire new type 5 CIs- Immediately perform a satisfactory BI spore test.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • - Chemical integrators were expired. Please ensure new Type 5 integrators are available and used. Expired integrators should be disposed of.
    • 29. Is the facility properly constructed and maintained to allow for the safe and sanitary provision of personal services (Non-Critical)?
      • - Sliding door leading into the reprocessing room was in disrepair. The door was stuck halfway open and hindered access to the room.- Drawer front located under the steam sterilizer (Tuttnauer EZ10) was missing.- Cover plates for electrical junction boxes, switches and outlets were missing throughout the facility.- Several garbage receptacles were full or overfilled in the washroom and tattoo rooms.Corrective actions:- Repair or replace the sliding door leading into the reprocessing room.- Repair the drawer front directly under the steam sterilizer. - Install cover plates for exposed electrical junction boxes, power outlets, and light switches.- Ensure garbage receptacles are emptied daily or more often if needed.

    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • -No hand soap was available on-site.-No alcohol based hand rub was available on-site*A staff member went to pick these items up and brought them back to the facility at the time of inspection. Handwashing supplies need to be available at all times.
    • 22. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Critical)?
      • - Chemical integrators were expired. Please ensure new Type 5 integrators are available and used. Expired integrators should be disposed of.

      • Physical and biological monitoring was conducted and results were documented, however, Type 5 chemical indicators have been expired since January 6, 2023.Please acquire new type 5 chemical indicators and ensure they are not expired.
    • 23. If the facility sterilizes equipment, are all proper sterilization, verification, and documentation processes in place (Non-Critical)?
      • Several packages of reusable critical equipment were found in the piercing room without load numbers and therefore had no sterility assurance.Please ensure all sterilized packages are labeled with the load number before they are moved out of the reprocessing area. Ensure they are either labeled on the plastic side, or outside of the seal on the paper side.
