      • Observation: Minor cockroach activity observed underneath the high temperature dishwasher.
      • Corrective Action(s): Implement pest control management practices to reduce and eliminate the activity of cockroaches. Keep areas clean and free of build up of water. Obtain sticky trap to reduce and eliminate the activities.

      • Observation (CORRECTED DURING INSPECTION): Repeat Violation:
      • Bleach spray bottle tested at 0ppm. No labelling for spray bottle, operator unsure when bleach was made.
      • Corrective Action(s): Bleach sanitizer must be available at all times at the concentration of 100ppm. Bleach spray bottle must be labelled to be easily identified. Solution of bleach should be replaced daily and checked to ensure a concentration of 100ppm of bleach is maintained. Operator produced a new solution of bleach at the correct concentration. Failure to comply in the future will result in further progressive enforcement.

      • Observation (CORRECTED DURING INSPECTION): Operator indicates that the hand wash station at the back kitchen is not working. Missing liquid soap from the handsink. Equipment and garbage blocking access to the handwash station.
      • Corrective Action(s): Remove all items from the hand wash station and provide liquid soap. Ensure all handwash stations are able to supply hot/cold water, liquid soap and paper towel available. Operator is currently using the 3 compartment sink for hand washing purposes. Operator obtained new liquid soap for the hand sink.

      • Observation: Dedicated handwash station in the kitchen area unable to provide hot water to the fixture.
      • Corrective Action(s): Replace/repair the fixture to ensure hot/cold water are able to run through the fixture.

      • Observation (CORRECTED DURING INSPECTION): Bleach spray bottle tested at 0ppm. Operator indicated that this solution was made few days ago. Operator made a new solution tested at 100ppm of chlorine. Ensure a new solution of bleach is made daily to ensure there is adequate sanitizing concentration.

      • Observation: Upright double door fridge:
      • Internal temperature of soup is 14.6C.
      • Water soaked noodles internal temperature 12.1C (Operator said this was placed in cooler this AM).
      • Surface temperature of all other items including meats, cut vegetables and noodles are between 9.0C to 12.1C.
      • Thermometer in the cooler shows 15C.
      • Corrective Action(s): Operator said he will put all items in the line coolers adjacent.
      • There are other additional coolers in the facility.
      • Only place uncut vegetables inside this cooler if the temperature does not go down.
      • Immediately remove all meats, soups, noodles and cut vegetables and place these in the other coolers.
      • Monitor the temperature using your thermometer. If the temperature does not go down to 4C in 30min to 1hr, call a professional to come and fix the cooler.

      • Corrective Action(s): Operator made during the inspection, thank you. Chlorine 100ppm.
      • Ensure this is kept ready for use.
      • Ensure that all surfaces are clean and sanitary for food perparation.

      • Paper towel at this hand wash station was not working.
      • This hand wash station was also blocked by garbage can and bag of recycling.
      • Hand wash station at the bar did not have paper towel dispensing properly.
      • Operator was preparing food with gloves at the time of inspection.
      • Corrective Action(s): Paper towel was re-supplied and properly placed the correct way during the inspection.
      • Ensure that both hand sinks are properly supplied at all times while food is being prepared.

      • Observation: Food items in upright cooler at 12C surface temperature. Items in the fridge >2hrs, internal temperature 10C.
      • Fridge display 8C.
      • Fridge last serviced Feb. 2022.
      • Left side items are personal use.
      • Operator said fridge display was 4C in this morning. No temperature logs, no thermometers for inside the fridge.
      • Other coolers are between -1C to 4C.
      • Corrective Action(s): Move all customer food items to working coolers.
      • Items with internal temperature >4C for >2hrs need to be discarded.
      • Call service technician to fix fridge.
      • Use a temperature log after the fridge is fixed to track temperature of fridge every 2hrs.
      • Put a working thermometer inside the fridge. Do not rely solely on the digital display on the outside.
      • Operator agreed to do this.

      • Customer foods inside measured between 10-12C. Customer foods are stored on right side, personal foods on left side. Storage of broth, 11.5C, meats and cooked noodles in water.
      • Corrective Action(s): Move all customer foods to working cooler. Operator agreed to move all items.
      • Monitor fridge to see if fridge cools down to 4C in 1-2hrs. If it does not, call service technician to fix cooler.

      • Observation (CORRECTED DURING INSPECTION): Basket of shrimp was stored on the counter next to dish wash area, 20C. Operator said this was defrosting, unattended.
      • Soup in container, 20C kept out during lunch for ease of reheating.
      • Items in cooler top >4C, stored during lunch for 2hrs. Cooler bottom <4C.
      • Corrective Action(s): Move items to cooler bottom and other cooler that is <4C.
      • Keep shrimp stored in coolers.
      • Discard if food items are being stored for more than 2hrs above 4C.
      • Ensure that all items are defrosted in cooler or under cool, running water, not out unattended at room temperature.
      • Operator moved all items to fridge <4C during the inspection.

      • Observation: 1. Back storage area bottom of door is still in need of a door sweep or leveling of door to ensure no pests can enter.
      • 2. Vent mesh holes (vent connects storage room to restaurant kitchen) are too large and may allow pest entry.
      • 3. Alley along property line between facility and adjacent building is filled with gravel, stones, soil, and garbage and recycling (milk jugs, wrappers and other debris). Evidence of mouse feces along alley.
      • Corrective Action(s): 1. Install a door sweep. Date to be completed by: 1 month.
      • 2. Replace vent mesh with finer mesh to prevent pest entry.
      • Date to be completed by: 1 month.
      • 3. Remove all waste and garbage around the facility.
      • Call a pest control contractor to do a thorough walk through of facility and around premises.
      • Install traps both inside and outside of the facility.

      • Observation: Top of chest freezers still have food/blood stains.
      • Non slip mat with holes below dry storage area has food debris, grime and some older droppings.
      • Grease under grill/cook area has improvement but still some grease on floors, walls, vents.
      • Corrective Action(s): Clean and sanitize chest freezers.
      • Change practice to maintain clean surfaces. Blood stains on top of freezer indicates meats were thawed or fresh and dripping going into the freezer. Please ensure you check packaging for holes. Reseal with bags. Reminder that cold food needs to stay cold <4C. Recommend to use bin or bus bin to transfer food in and out of the chest freezers so food is not stacked on top of freezers.

      • On going maintenance for floors, walls, vents is needed for grill/cook area to prevent pests and maintain sanitation. Please adjust your Cleaning Plan and inform your staff.

      • Observation: Rodent feces observed along window sills, along the entire length of the front window.
      • No rodents seen during the inspection.
      • Two customer seating areas in this section of the restaurant.
      • Feces observed behind the small drink display cooler.
      • Feces observed in the corners of the Men's Washroom.
      • Corrective Action(s): Operator said some feces in the back corners are old.
      • Note that rodent feces and urine attract other rodents.
      • All feces need to be cleaned with bleach solution and wearing mask and gloves.

      • Put all dry storage items in the closet AND in the kitchen in plastic, lidded containers.

      • Observation: Back chest freezer and equipment storage area is cluttered and there are some candy wrappers on the floor.
      • Small amounts of rodent feces in this area.
      • Large gap between locked door (entrance to storage) and floor.
      • Vent has large size mesh that may allow entry into restaurant.
      • Floor has crack along floor boards that may allow entry through wall and opening around drains of 3 comp sink and the floor on the other side.
      • Corrective Action(s): Clear and clean back storage room; clean up any rodent feces by next week.
      • Remove unnecessary items and clutter or any harbourage materials.
      • Replace vent mesh with finer mesh by next week.
      • Seal all cracks and/or install firm baseboards. Put a rodent resistent sealant along baseboards and cracks or temporarily put steel wool in holes and cracks.
      • Install a firm, rodent resistent door sweep.

      • Observation: Food debris, grease and grime visible along front service area behind hand sink, garbage and recycling bin.
      • Grease behind grill area on the floor.
      • Grease along the walls and visible grease on the ventilation fume hood.
      • Fallen whole onions, garlic peels and noodles in various places especially under grill lines.
      • Dry storage area:
      • Mat in front has food debris that can attract rodents.
      • Some spilled food debris and bags piled at the bottom of this closet.
      • Mop sink area:
      • Sink has grime that needs cleaning. Room is cluttered and difficult to access for inspections and cleaning.
      • Under 3 comp sink and line cooler:
      • Dirt and grime build up that needs cleaning.
      • Corrective Action(s): Deep clean all areas mentioned with soap and hot water.
      • Thorougly sanitize all areas that we went through during the inspection that appeared to have rodent feces.
      • Use a commercial degreaser to remove grease.

      • Corrective Action(s): Make sanitizer and immeditely clean and sanitize food surfaces and especially before dinner food prep. Corrected during inspection. Sanitizer made 200ppm bleach solution.
