    • 16. Is the facility free of unapproved animals, vermin and/or insects (unless noted elsewhere), and maintained to prevent the entry or harbourage of pests?
      • Observed a nymph German cockroach in the glue board underneath the dishwasher. Other glue boards were empty. Consult with the hired pest control company to detect early signs of an infestation and coordinate with them to determine a plan to abate the issue.
    • 22. Are utensils and equipment maintained in a condition and a manner that ensures the safe handling of food?
      • Soiled cardboard boxes were used to store the pizza pans. Food equipment must be stored in a manner that protects it from contamination. Refrain from using cardboard boxes as the material is not smooth, durable, or easy to clean. When cardboard boxes are stored directly on the floor, moisture may seep into the material and contaminate the clean pizza pans stored inside. Replace the cardboard boxes with a material that is smooth, durable, and easy to clean.

      • 1. There was build up on grease and food debris underneath the grill area. Ensure hard-to-reach areas are regularly cleaned to remove build-up of food and to prevent the attraction of pests. Thoroughly clean the indicated area.2. There was dust accumulation on the ceilings above the food preparation sink. Ensure the ceilings are regularly cleaned to remove debris and dust to ensure contaminants do not fall on food that is prepared below.
