      • Written procedure templates for skin cleansing and accidental injuries were onsite but not complete. Please fill out page 2 for manicure, pedicure and waxing services. Please review/sign page 5.

    • 08. Is hand hygiene performed by the personal services worker when required, in a proper manner and are provisions available to perform hand hygiene?
      • There was a pair of used gloves hanging on the drawer of nail station. The staff member indicated that she would wear the same gloves while carrying garbage.The gloves are single used item and must be discarded after single use.The staff member discarded gloves on site.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • The alcohol-based hand sanitizer bottles on the nail stations were expired. Please replace these with new alcohol sanitizer bottles.
    • 20. Is reusable equipment disinfected properly?
      • There were no HLD (High-Level Disinfectant) log sheet in place.HLD log sheets were provided to the facility. A fresh solution was made and tested with a pH strip that showed "Pass" result. This test was populated in the HLD log sheet during inspection.
    • 20. Is reusable equipment disinfected properly?
      • There were some uncleaned tweezers found in the waxing room trolley that had oil and wax sticked to it. Please ensure that the non-critical waxing tools (such as tweezers) are properly cleaned and disinfected with a minimum low-level disinfectant.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The facility was not maintained in visibly clean state. The client services areas had dust over the tables. Nail machines were not cleaned from inside. Please maintain the cleanliness of the facility.
