      • Not in Compliance:Facility written procedures for "cleansing skin" is incomplete. The name of the cleansing product is not included.Corrective Action:Complete the written procedure for "cleansing skin" and provide a copy.In Compliance:Facility has written procedures for "client care in the event of an accidental skin cut or puncture"

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • Facility does not have written procedures for:- the cleansing of skin and mucous membrane- client care in the event of an accidental skin cut or punctureCorrective Action:Provided operator the AHS written procedures templates to review and complete.
    • 10. Is cleansing and antisepsis of skin and mucous membrane performed in accordance to written procedures and are suitable products being used?
      • Upon reviewing waxing procedures with staff, it was discovered that there is no skin cleansing step prior to waxing.Upon reviewing waxing procedures with owner, it was noted that baby wipes are used to cleanse the skin area prior to waxing. Corrective Action:Advised Owner to re-educate staff regarding conducting skin cleansing with an appropriate skin cleanser product prior to waxing the area.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Wooden applicator sticks were being re-used and stored in product containers (including product containers for pedicures and Vaseline container in wax room)Corrective Action:- Directed staff to discard of wooden applicator sticks, and re-fill containers with fresh product.- Educated staff to only use wooden applicator sticks one time and then discard and to not store them in the product.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • 1) Single-use items such as foam files were being used more than once.Corrective Action:Directed and educated staff to not use single-use items more than once.2) Foam toe/finger separators were observed in the dryer. These have been washed then dried via on-site laundry.Corrective Action:Directed and educated staff to refrain from this practice and to discard these as they are not easily cleanable, cannot be properly disinfected, and are single-use items.

      • 1) Facility does not maintain a log for high level disinfectant (PREempt CS20). Corrective Action:An AHS high level disinfection log was provided for use.2) Test strips for high level disinfection were expired as of (04/30/2024).Corrective Action:Purchase new test strips for high level disinfectant.
    • 20. Is reusable equipment disinfected properly?
      • 1) One bottle of high-level disinfectant (PREempt CS20) was observed to be expired (as of Feb. 8, 2025). However, it was not yet opened or in use.Corrective Action:Advised operator to not use this disinfectant and remove from premises.2) Cleaning and disinfection of the pedicure footbath was observed to be incorrect. Staff removed the liner and dismantled the impeller. However, staff proceeded to clean/disinfect the footbath with water and then RTU with a contact time of under 10 seconds.Corrective Action:Educated and advised staff of proper cleaning and disinfection procedure for pedicure footbaths:1. Clean (wash and rinse) the basin and impeller parts (with a cleaner product, or the PREempt RTU which is a cleaner and disinfectant).2. Disinfect (apply disinfectant) and allow to remain on surfaces for the required contact time (3 minutes for PREempt RTU), then rinse/dry.(Follow manufacturer instructions for any product/disinfectant to be used.
    • 26. Are cosmetic products appropriate for their intended use and being handled properly?
      • One container of wax (on a warmer) is not stored covered.Corrective Action:Ensure wax is stored covered.
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • Observed staff use a wet cloth to clean and disinfect pedicure chair surfaces (cushion, seat, armrests, exterior of tub, garbage bin, pedicure product cart, etc.).Corrective Action:Advised staff to first clean these surfaces then use at minimum a low-level disinfectant to disinfect these surfaces (such as lysol/clorox disinfectant spray, or the PREempt RTU which is an intermediate level disinfectant).
    • 27. Is the facility maintained in a visibly clean state and are environmental surfaces cleaned and disinfected properly?
      • The dresser and wax holders in the waxing room are not maintained in a visibly clean state.Corrective Action:Clean these surfaces and ensure they are maintained in a visibly clean state.
    • 32. Are general nuisance conditions, other than those noted in other violations, being prevented at this personal services facility (Critical)?
      • Upon entering the facility for inspection, the door was observed to be kept half-way open. Staff mentioned this was to help with bringing in fresh air and to ventilate the chemical odours.Corrective Action:- I advised the staff to keep the door closed as leaving the door open introduces the potential for entry of contamination such as dust, debris, insects, etc.- Staff closed the door and turned on their mechanical ventilation.

    • 03. Are facility-specific written procedures established to adequately describe safe and sanitary practices?
      • No written procedures for skin cleansing prior to services and accidental cuts were available. Please fill out the procedure templates and email the copy to our office.
    • 13. Is single-use or uncleanable equipment being used as intended (Critical)?
      • Used nail files and sanding bands were observed on the manicure working stations. The single-use tools must be discarded after being used to prevent the re-use of the single-use tools and contamination of clean tools. Addressed during the inspection.
    • 20. Is reusable equipment disinfected properly?
      • The high-level disinfectant used for tools disinfection was not at proper concentration - tested with test strips during the inspection. The solution was changed during the inspection.

      • The tools were not fully immersed in the high-level disinfectant solution during the disinfection. Ensure that the tools are fully immersed.
    • 20. Is reusable equipment disinfected properly?
      • No records for testing the concentration of the high-level disinfectant (Preempt CS20) were available. The disinfectant testing log was started during the inspection.
