      • Observation: A remote inspection of this premises was conducted by phone on DD-MMM-YY. The operator completed a Food Premises Self-inspection Checklist and submitted this along with any requested supporting documents to the environmental health officer. An on-site inspection was not conducted by the environmental health officer.
      • Corrective Action: N/A

    • 209 - Food not protected from contamination [s. 12(a)]
      • Observation: Poison mouse bait was observed in the kitchen.
      • Corrective Action: Remove poison bait immediatly and refrain from using as mice can carry poison on their fur and feet, contaminating food packaging, food itself and or food contact surfaces.

      • Observation: Foods were left uncovered and open to contamination. Ice cream containers were observed in a chest freezer, uncovered, with heavy build up of frost around the inside of the freezer. (Public Health significance) - Contamination of food from contact with objects, people, pests or chemicals can cause serious illness.
      • Corrective Action: Cover or otherwise protect all foods from airborne contaminants, drips from raw foods, and contact with objects and staff. Unthaw chest freezer to maintain in a sanitary condition
    • 301 - Critical Equipment/utensils/food contact surfaces not maintained in sanitary condition [s. 17(1)]
      • Observation: Vent above the wok was observed to have a heavy build up of grease.
      • Corrective Action: Increase cleaning frequency to limit oil build up- that can drip onto food being prepared, contaminating it
    • 302 - Critical Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation: No active sanitizer detected in sanitizer containers or spray bottles. Bleach is used as a sanitizer, however the operator added soap to the container, reducing the efficacy of the bleach (Public Health Significance) Sanitizing solutions must be strong enough to destroy bacteria on surfaces to prevent the contamination of staff, clients or food on contact with the improperly cleaned surface.
      • Corrective Action: Change sanitizing solutions as required to always maintain an effective concentration (200 ppm unscented bleach, 200 ppm QUAT)- follow manufacturer's specifications). Only use bleach + Water as your surface sanitizer
    • 304 - Premises not free of pests [s. 26(a)]
      • Observation: Evidence of mice; droppings were noted in the facility. Walk in cooler was observed to have quite a few mice drppings on the floor in the back right of the fridge . Droppings were also observed outside the walk in cooler underneath the shelving unit.(Public Health Significance) Rodents and other pests may cross contaminate foods and surfaces or contaminate human food with feces and urine, carry parasites and other diseases.
      • Corrective Action: Remove any pest attractants such as food and water, eliminate access to the building and routes they use inside. Trap and remove the pests. The use of a professional pest control operator is recommended. Clean and sanitize area where droppings are located and monitor situation.

    • 209 - Food not protected from contamination [s. 12(a)]
      • Observation: (CORRECTED DURING INSPECTION) Various implements used a scoops are left in product for ongoing use including ingredient bins, ice machines, ready to eat product. Observed scoops within flour products. (Public Health Significance) Improper scoops and scoop storage exposes items to bare hand contact and potential contamination.
      • Corrective Action: Store scoops so that foods are not exposed to the handles or bare hand contact. Store food scoops in a sanitary manner and clean and sanitize them regularly.
    • 209 - Food not protected from contamination [s. 12(a)]
      • Observation: Observed egg rolls, and dumplings stored within cardboard containers within the walk-in refrigerator and freezer. Contamination of food from contact with objects, people, pests or chemicals can cause serious illness.
      • Corrective Action: Operator to remove all cardboard used to store produce and organic food items from the walk-in refrigerator and / or freezer and purchase food grade containers with lids to protect food products from potential sources of contamination.
    • 315 - Refrigeration units and hot holding equipment lack accurate thermometers [s. 19(2)]
      • Observation: No accurate indicating thermometers were observed within the walk-in refrigerator. Accurate indicating thermometers ensure proper cold-holding storage temperatures of 4 C in refrigerators and -18 C in freezers, respectively, to reduce the growth of bacteria which can contaminate food products and result in potential foodborne illness.
      • Corrective Action: Operator to purchase, install and ensure accurate indicating thermometers that are in good working order to ensure potentially hazardous foods are stored at the proper cold-holding temperatures of 4 C and -18 C respectively.
