- Both the men and women's bathrooms did not have soap. Staff corrected this during the inspection. Soap must be provided at all times to allow for proper handwashing.

- Walk in freezer had ice built up and water was observed leaking on the outside of the freezer. Please ensure the leak is repaired and the ice is removed.

- The plastic covering for the metal cutting board, in the sandwich prep area, was in disrepair. This was corrected during the inspection. Staff member threw out the plastic. Please ensure that food equipment is cleaned and replaced as needed to prevent physical and bacteriological contamination.

Food debris was observed under shelves, equipment and in the corners by the baseboards throughout the entire kitchen.These areas must be routinely cleaned.Area under the shelves in the walk-in cooler had food debris built up. Please ensure these areas are cleaned.

- The dishwasher was dispensing 0PPM chlorine sanitizer at the time of inspection, because the feeder tubing hadn't been pushed down enough. Use chlorine test strips at the beginning of every shift so these issues can be rectified before daily dishwashing.
