- Corrective Action(s): Ensure to remove droppings and sanitize affected areas every morning. Set traps throughout - no poison inside of the facility. Increase pest control inspection frequency.

- Observation (CORRECTED DURING INSPECTION): Bleach sanitizer solutions for surface sanitizing measured out of range. One bottle was below 100ppm and two bottles were above 200ppm.
- Corrective Action(s): Staff corrected all solutions to measure between 100-200ppm chlorine. Facility has test strips on site - use these strips to verify the concentration each day before use.

- Observation: Significant presence of flies were observed in the bar area.
- Corrective Action(s): Remove the recycling box from inside the facility to the hallway outside of the kitchen. Facility must receive a visit by a professional pest control company for the month of June. Service must be done for flies and rodents.
- Forward the report to the EHO for review by the end of the month if the visit is taking place after the follow-up inspection.

- Observation: Rodent droppings observed in the following areas:
- > In the main kitchen - under the white counter directly across from the doors, on every shelf that is under the counter, in the crevices between the fridges and counter
- > In the bar area - in the crevice between the dishwasher and counter, in the storage spaces under the back counter, on the shelves against the west wall.
- > Behind the counter that is to the right of the front door.
- Corrective Action(s): Remove droppings. Clean and sanitize the premises as per your sanitation plan.
- > Maintaining a sanitary facility will prevent the precense of pests and assist with monitoring for new activity.
- All items stored on the shelves must be removed for a thorough cleaning. Any unused items must be discarded.
