- Opening at the base of the back door and receiving garage door that will allow mice and other pests to enter the premises.
- Corrective Action(s):
- Install a door guard/sweep on the base of the back door and receiving garage door.

- Observation:
- 1) Dust accumulated on the ceiling tiles around the air diffuser, above the dishwasher. The dust can fall into food and contaminate it.
- 2) Accumulated food and dust on the wire storage racks in the walk-in-cooler. This will attract pests.
- Corrective Action(s):
- 1) Clean the ceiling tiles to remove the dust. Add this to the Cleaning Schedule, so it is done on a regular basis.

- Observation:
- High temperature dishwasher had a rinse cycle temperature of 67.9C at the plate's surfaces, as per max/min thermometer. The temperature must be >71C at the plate's surface to sanitize and kill germs. During the inspection the operator called a service company.
- Corrective Action(s):
- - Immediately inform staff they must manually sanitize dishes by soaking in a sink of sanitizer (Quats).
- - Increase the rinse cycle temperature, at the booster, so it is above 82C and the plate surface temperature is above >71C.
- - Purchase a waterproof thermometer to place in the dishwasher, to monitor the temperature during the rinse cycle.
- - Contact our office when the dishwasher is operating correctly, so a follow-up inspection is completed.

- Observation: Gap under the receiving bay door (roll-up) is sufficient in size to allow mice to enter the premises.

- Observation: Dishwasher is still not reaching the minimum required 71C at final rinse.
- Corrective Action(s): Have dishwasher adjusted/repaired so that it can maintain this minimum at all times. Due: Oct 23, 2023
- In the mean time, continue to manually sanitize the dishes after they come out of the dishwasher.

- Observation: Met with owner on site to discuss dishwasher and it was determined that the dishwasher was recently added and a sink removed. This was done without consultation with Health. The minimum requirement for dishwashing in the presence of a dishwasher is two sinks. Two sinks still on site however are used for food preparation.
- Corrective Action(s): EHO to re-look at originally approved plans and will advise what will be required to meet guideline intent.

- Observation: High temperature dishwasher only reaching 64-67C when it must reach 71C at minimum. Not a high volume of dishes go through the dishwasher, a lot of equipment is clean in place or done by hand because its too large to fit in dishwasher.
- Corrective Action(s): Adjust/repair dishwasher so that it can maintain 71C at all times. Due: Oct 18, 2023
- Dishwasher may continue to be used but dishes must be manually sanitized by using spray sanitizer or submersing in sanitizer in sink.
- Notify undersigned EHO when repair is complete.

- Observation: Multiple large containers of frozen chicken thawing at room temperature in warehouse area. Staff stated they take the frozen blocks of chicken out of the box then move them to plastic containers at the start of the day then move them into the cooler at the end of the day to finish thawing.
- Corrective Action(s): Thawing at room temperature like this can allow for bacteria to grow on the parts of the chicken that thaw the fastest (corners) while the center is still frozen. Staff stated that thawing in the cooler takes too long. Thawing can and must be done in a safer manner. Recommend the cold water bath method where the frozen chicken would be submersed in water (can also be done in a large plastic tub) which transfers the cold faster than air and in a safer manner.

- Observation: Back bay door was up approximately 1 meter which is more than enough space for pests to enter with no one actively going in/out of this door. Staff stated that they just got a delivery.
- Corrective Action(s): Ensure the doors are closed fully when not in use to prevent pest entry.

- Observation: Observed a gap on the right side of the bay door next to the back exit door. Also observed a few gaps under the back exit door. These gaps could allow pests such as mice or rats to enter the building. Some mouse droppings observed in the back warehouse near these doors.
- Corrective Action(s): Improve seal in both of these doors to prevent the entry of pests. Due: 1 week (March 14, 2023)
