- Additional procedures / record keeping necessary for testing concentration of disinfectant (daily) for manicure/pedicure tools are required. High level disinfectant being used appropriately for manicure/pedicure instruments; however, process is not in place to check/record concentration of disinfectant solutions (test strips are on site). Ensure monitoring is completed daily.

- Clearly separate tool cleaning and handwashing (ie. one sink for each process) or provide policy for cleaning/sanitizing sinks between different uses.

- No test strips were observed for high level disinfectant for semi-critical instruments. However, operator produced an invoice with test strip order information. Ensure test strips are available at all times when operating.

- Ceiling tile missing and stained tile by front entrance to treatment area indicates issue with leak. Remove stained tile, repair any source of leak, and monitor for addtional issues.Ceiling tile in back treatment area (above pedicure equipment) is stained indicating issue with leak. Remove stained tile, repair any source of leak, and monitor for addtional issues.
