- Sour cream has mold present inside the container. The best before date was 2 months past. The product was dicarded.
- 11. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Critical)?
- The dishwasher is not meeting the 100ppm chlorine sanitizer concentration. The mechanical system is pumping the sanitizer into the dishwasher. Action shall be taken to replace the sanitizer stock chemical. All dishware shall be sanitized in a 100ppm Chlorine solution after washing them in the dishwasher.
- 12. Are there adequate and properly operating dish washing facilities to effectively clean and sanitize equipment and utensils (unless noted elsewhere)(Non-Critical)?
- Test strips to test the dishwasher sanitizer and surface sanitizer is not available. Test strips shall be obtained to ensure the sanitizers used in the facility are at an acceptable level on a daily basis.
- 13. Are properly operating hand washing sink(s) accessible to food handlers, located in suitable areas, and stocked with appropriate hand washing supplies?
- Paper towels are missing from the dedicated hand sink. Action shall be taken to stock the hand sink with paper towels.

- The storage in the single use equipment in the back room has recyclable containers beside the containers. Action shall be taken to organize the area to prevent cross contamination.
- 23. Is the facility maintained in a clean and sanitary condition?
- The walk-in cooler has a buildup of debris on the walls. Action shall be taken to clean the walls.
