     • Facial waxing services were offered without notification to this office. If the facility permanently closes, moves locations, adds new services or uses new beauty instruments, please ensure this office is notified by completing a personal service notification form on-line.

     • Facility-specific written procedures were completed during the inspection and left on-site.

     • 1. Barbicide glass jars were visibly soiled and/or half-filled. Ensure barbicide solution is made fresh daily according to label instructions. Hair-related tools must be washed with soap/water and dried prior to being disinfected for 10 minutes in barbicide. Rinse tools after immersion in barbicide prior to use on client. If needed, purchase a larger inexpensive container to allow for full immersion of large brushes/combs.2. ILD (intermediate-level) disinfectant for electric hair clippers was missing a DIN. Obtain a suitable type of disinfectant. The ILD must have the following claims on the label: -DIN -disinfectant -tuberculocidal OR mycobacterium
