    • 205 - Cold potentially hazardous food stored/displayed above 4 °C. [s. 14(2)]
      • Observation: A) Prep cooler in kitchen clam shell lid and top of unit warm to touch. This is a result of two issues: i) the pass through warmer element pushes out a lot of heat extending towards cooler; ii) the clamshell lid, when left open, is subject to more of this radient heat and ends up warming the entire top of unit. Temperatures in the inserts 6 to 8 C. Below in unit 3 C so the unit can work properly.
      • B) Seafood cooler/salad prep cooler in front not working. Seafood removed and discarded according to chef. Some foods remain (mostly related to salad prep) and supplemented with ice. Temperatures okay. Guacamoli discarded as it was not under temperature control.
      • C) Use of ice to supplement cooling of some of the foods not adequate. Ice is sparcely used. At minimum make sure the food product/inserts are always touching or swimming in the ice bath. Replace with fresh ice multiple times during the day.
      • Discussed all of above with chef.
      • Corrective Action(s): A) Turn down passthrough warmer element. Place sign on clam shell lid to keep closed.
      • B) Have seafood cooler serviced ASAP. Do not keep any seafood in this unit until repaired. Keep moderate risk sauces etc. for salads under ice supplement until unit is repaired.
      • C) Make sure to increase the volume of ice used for ice baths when supplementing temperature controls as discussed.

      • Observation: Several handwash stations not adequately maintained:
      • - Downstairs prep kitchen - sink taps turned off. Faucet also stripped and not functional.
      • - Downstairs employee washroom paper towel dispenser broken (does not feed properly).
      • - Bar area lacking paper towel dispenser or similar set up. Staff using wiping cloths to dry hands.
      • Corrective Action(s): Fix all handwash stations and accesory equipment such that there is hot and cold running water and there is always liquid soap and paper towels available for use.
      • Have handwash stations reviewed and serviced as discussed. The front bar area just needs a paper towel bar proximally located for access and use.

      • Observation: Ceiling tile remains missing and/or of improper design in the basement prep area. Several sections are not cleanable. Food prep space directly below exposed drain lines that may periodically leak. Existing tile appear either wet or stained.
      • Corrective Action(s): This is a repeated issue and may lead to an escalation of enforcement, including issuing a violation ticket if not addressed in noted timeline. Replace ceiling tile with washable tile to allow easy cleaning and protect food from dripping wastewater. Follow up inspection in ten days.

      • Observation: The following areas were observed with build up of food debris. These include:
      • - floor behind cookline (grease etc.); minor.

      • Corrective Action(s): Clean up identified areas. Discussed with chef. Review at follow up inspection.

      • Observation: Fruit flies seem reduced, but still a problem in the bar area. Noted most of the beer taps have been capped and covered with plastic wrap, but a couple of taps remain exposed. These taps are leaking at tap, but also at the inner hose connection. Flies noted where build up of sludgy beer evident.
      • Corrective Action(s): Continue to target this area for improved cleaning and stop leaks to eliminate some of the food sources. The taps need better daily cleaning on top of plastic wrap barrier.

      • Observation: The bar looks much cleaner. The kitchen cubby area is much cleaner.
      • **The basement is not clean. Food debris on floor in basement food prep area, walk-in cooler floor and behind equipment. Shelves in walk-in cooler are laiden with debris and mould.
      • Corrective Action(s): Target the basement for cleaning and improved maintenance as per other parts of the premises. If the basement walk-in can't be better maintained, food storage will not be allowed in this unit. Follow up inspection requireed, see report for date.

      • Observation (CORRECTED DURING INSPECTION): Rice block in kitchen cubby prep station observed with several fruit flies crawling around and possibly into food. Discussed with manager and it was agreed the product would be weighed and discarded.
      • Corrective Action(s): Product discarded.

      • 302 - Equipment/utensils/food contact surfaces not properly washed and sanitized [s. 17(2)]
      • Observation (CORRECTED DURING INSPECTION): Sanitizer measured at 0 ppm chlorine. Transport line empty. Repositioned sanitizer container, adjusted feed line in container and primed system. Sanitizer was observed in dishwasher at very low level (10 ppm?). Concentrate looked light in colour. Switched out concentrate container and continued to pirme system. Measured at 100 ppm.
      • Corrective Action(s): Monitor dishwasher sanitizer residuals and ensure minimum 50 ppm maintained. M<ake sure to tell staff to keep sanitizer container closest to machine as it has the shortest transport line of the three chemicals.

      • Observation: Fruit flies noted in several areas of premises including: Basement food prep room (minor), bar area at and under right side ice bin, in cubby prep station in main kitchen area, and around dishpit (moderate). Syrups, stagnant water and dirty drains visible in the noted breeding areas.
      • Corrective Action(s): Begin process to clean up premises and remove any and all viable food sources for fruit flies, Target all floor drains and areas with water accumulations with chemicals and hot water on a daily basis until breeding cycle is broken. Discuss trapping options with Pest Control and as discussed.

      • Observation: Several areas of the premises observed unsanitary and/or with excessive accumulation of food debris including:
      • - basement food prep area and walk-in cooler floor;
      • **Ceiling accoustic tile also observed mouldy and stained. (replace with proper washable tile).
      • - Main kitchen in the cubby area prep station - floor under shelving a mess with food debris and significant stagnant water (likely a primary fruit fly breeding area);
      • - bar area at and under right side ice bin (stainless steel with caked on syrup or other liquor). Also, many glass mats observed with build up of dust and debris.
      • - Floor drains dirty?
      • - Cutting boards in kitchen stained with food and are generally discoloured.
      • Corrective Action(s): Identify above areas as priority cleaning zones and re-establish better and consistent cleaning practices with staff.
      • Target and fly areas of accumulation and provide improved cleaning, including floor drains as noted.
      • Follow up inspection needed and noted on report.

      • Observation: Fruit flies noted to varying degrees in different areas of premises. HEAVY activity noted at glasswasher area of bar and surrounding area. Minor activity in basement and steps leading from basement. No activity in staff washroom. Discussed with staff, leak from bar to basement may have created breeding opportunity in floor /ceiling. Pest control planned bombing to take place.
      • Corrective Action(s): Save Pest Control report for review at follow up inspection. Seek advice from Pest Control on continued fly population control.
      • Remove all areas of stagnant ponding water as discussed.
      • Review at follow up inspection - date noted on report.

      • Observation: Bar area around glasswasher seems to leaking or bleeding onto the floor and into har-to-reach places and is subject to significant fruit fly activity. Hand/dump sink at glasswasher has no access to plumbing. Interceptor pan below glasswasher not adequately containing water or splashes, contributing to fly breeding environments. It seems cleaning and removing bar debris is not sufficient as debris was observed.
      • Shelves for glasses liquor painted (matt black) and deteriorating. Surface is either flaking paint or noted with significant amounts of dust, including on the plastic bar mats themselves. This was noted in previous inspection.
      • Corrective Action(s): Shelves should be resurfaced with light coloured material/paint. This will make it easy for staff to see when surface requires cleaning, outsie of normal cleaning schedule. The bar area requires an improved cleaning protocol.
      • Water run off around and from glasswasher must be improved and fly population controls put in place.
      • Provide easy access to plumbing in this section as discussed.

      • Observation (CORRECTED DURING INSPECTION): Many sauces kept at room temperature or in suis vide machine at approx. 40 C. Discussed with chef. Current process is to use and rotate with fresh sauces from cooler. And continue with that cycle. ONLY ACCEPTABLE IF SHELF STABLE PRODUCT.
      • Corrective Action(s): Chef will control sauce volumes for each rush and discard at end of rush if not shelf stable products. Review food safety plan and confirm by email verified processes.

      • Observation: Right handsink along cookline lacks barrier to protect food from splashes in adjacent prep cooler.

      • Observation: Minor volumes of fruit flies are evident in the bsement, main floor kitchen and at bar.
      • Corrective Action(s): Staff must address/remove all standing water around work areas subject to any version of water activity on daily basis.
      • - Drains must be cleaned weekly and spot chemical application with hot/boiling water daily.
      • - Ponding water at all sinks must be removed daily.
      • - garbage cans and areas where garbage is placed, including at the dishpit must be thouroughly cleaned at the end of the day.
      • - Fruit and/or residual alcohol must be cleaned up throughout the day and nothing should be left on the floor on or under the counters at the end of the day. All this is food and breeding habitat for these pests.

      • Observation: Fruit flies noted in basement storage areas including:
      • - Floor drain by ice machine
      • - Beverage syrups dispensing station
      • - prep room, including ceiling/sub-ceiling in and around pipe network.
      • Minor rodent activity noted with droppings on floor and gnaw marks on some base boards and paper on floor in noted areas above.
      • Minimal fruit fly activity noted in main kitchen and bar area.
      • Corrective Action(s): The basement requires a complete clean up in above noted areas. Remove wastewater buckets or similar from below water drain lines and repair plumbing network where leaks occuring. Clean any exposed floor drains or similar drainage networks (including within false ceiling cavity in basement). Clean and remove any stagnant water on floor of premises. Implement plan to clean and maintain all floors and drains subject to accumulation of water. Review plan in detail at follow up inspection date noted on report.

      • Observation: Floor drains in basement and main floor extremely dirty and observed biomat (breeding environment) readily available for fruit flies to use. Baseboards in several areas of basement observed as potential access points for rodents. Plumbing networks and drains observed as potential fruit fly breeding habitat. Pictures taken.
      • Corrective Action(s): Corrective action must include elimination of biomat build up, repair of existing waste water leaks from ceiling pipes, repair of walls and replacement of damaged ceiling tile with proper and washable ceiling tile in basement. Plan of corrective action and repair, and sanitation plan required within 30 days.

      • Observation: Following areas observed dirty and require improved sanitary maintenance:
      • - Basement: floor everywhere, beverage syrups station, behind bin freezer (must move to clean), under and around linens shelf, at floor drain and around ice machine and hot water tank;
      • - walk-in cooler floor;
      • Corrective Action(s): Clean above areas described above. Repair walls, ceiling structure and flooring as required to allow necessary cleaning as discussed. To be reviewed along with action plan on follow up inspection dated noted on report.

      • Observation: Floor in walk-in cooler observed significantly rusting in all areas below shelves. Floor is extremely dirty and under-maintained, especially along floor wall interface. Discussed with chef. This is a repeated and long standing issue and continues to impact sanitation of this environment. Food observed stored directly on floor, which presents cross contamination issues.
      • Corrective Action(s): Immediately get food or food containers off floor. (Done during inspection). Develop a repair plan for walk-in cooler to allow this unit to be kept clean and sanitary. Corrective action plan must be submitted within two weeks and approved by this office with completion date no longer than 120 days or 4 months from this inspection date.
